Quick answer
Most anal itching comes from moisture, small traces of stool, harsh wiping, soap or scented wipes, and the scratch-itch cycle, often alongside hemorrhoids. Rinse with water, pat dry, skip soap and wipes, wear cotton and stop scratching. A short course of 1% hydrocortisone helps many people. See a doctor if it lasts more than a few weeks.

Key takeaways
- Reviews of pruritus ani name stool residue on the skin and food irritants as the main primary causes. Hemorrhoids, fissures, skin conditions, infections and, rarely, cancer are secondary causes.
- Hemorrhoids cause itching: the NHS lists an itchy anus as a main symptom of piles.
- In a small placebo-controlled trial, 1% hydrocortisone ointment cut itch scores by 68 percent compared with placebo over 2 weeks. Hydrocortisone for piles should not be used for more than 7 days without a doctor's advice.
- Pinworms are a common cause of night-time anal itching in children, and a tape test on three mornings finds about 90 percent of cases.
- For itch that will not go away, capsaicin ointment and methylene blue injections have helped in small studies, under a doctor's care.
On this page
An itchy anus, called pruritus ani by doctors, is one of the most common and least discussed bottom problems. It is usually not a sign of anything serious. Most of the time the skin around the anus is reacting to moisture, tiny traces of stool, rough wiping, soap or scented wipes, and then to scratching, which makes the itch worse. Hemorrhoids are a frequent partner, because they leave the area damp and harder to clean.
This page covers the common causes, a simple 2-week plan that settles most cases, what to use from the pharmacy, and the signs that mean you should see a doctor. If the itch comes with a lump or bleeding, read about external hemorrhoids and bleeding hemorrhoids as well.
What causes an itchy anus?
A review of pruritus ani describes it as common, with many causes, mostly problems of the anus and rectum 1. Doctors split the causes into two groups 2:
| Type | Causes |
|---|---|
| Primary (no other disease found) | Stool residue on the skin, food and drink irritants, moisture, over-cleaning |
| Secondary | Hemorrhoids, fissures and other anal problems, skin conditions, infections including sexually transmitted ones, systemic diseases, and rarely cancer |
Moisture and stool residue
The skin around the anus is folded and stays warm and damp. Small amounts of stool or mucus left behind after a bowel movement irritate it. Reviews name this soiling as one of the two main primary causes 2. Loose stools, sweating, tight synthetic underwear and long days sitting all add moisture.
Hemorrhoids and skin tags
The NHS lists an itchy anus among the main symptoms of piles, along with bleeding, mucus and pain 4. Surgeons note that hemorrhoid tissue that slips out of the anus can cause significant irritation and itching 5. Skin tags, the soft flaps left after old flares, can make cleaning harder, which keeps the itch going. More is in anal skin tags.
Soap, wipes and scratching
Harsh soap, scented or alcohol wipes, and scrubbing with dry paper strip the skin. Scratching then breaks the surface, and broken skin itches more. A narrative review of anal itching advises avoiding moisture and soap in the area as a first step 3.
Food and drink
Some people find certain foods and drinks set off the itch. Reviews list food irritants as a primary cause and suggest avoiding suspect foods 23, but which foods matter varies by person and the evidence is thin.
Pinworms
Pinworms, called threadworms in the UK, are tiny worms that live in the gut. They affect about 30 percent of children worldwide, and the most common symptom is anal itching at night 11. The eggs spread from the bottom to the mouth on hands and surfaces, so other people at home can be affected too 11. A tape test, pressing clear tape to the skin around the anus first thing in the morning, finds about half of cases on one try and about 90 percent when repeated on three mornings 11. Treatment is a medicine such as mebendazole 11, so ask a pharmacist or doctor.

How to stop an itchy anus: a 2-week plan
The general advice from reviews is to remove irritants, stop the scratching, and keep the skin clean and dry 13. These steps settle most cases within a couple of weeks.
- Clean with water only. After a bowel movement, rinse with a bidet, a peri bottle or a damp cloth. Skip soap, scented wipes and baby wipes on the area 3.
- Pat dry gently. Do not rub. A soft cotton pad or toilet paper is fine for patting.
- Protect the skin. A thin layer of a plain barrier ointment, such as zinc oxide or petroleum jelly, keeps moisture off.
- Wear loose cotton underwear and change it if it gets damp.
- Stop scratching. Keep nails short, and wear thin cotton gloves at night if you scratch in your sleep.
- Keep stools soft and regular with fiber and water, so you wipe less. The narrative review also suggests more fiber 3.
- Try cutting one food or drink you suspect for 2 weeks, then add it back and see.
Bidets and rinse bottles make the water-only cleaning easy. What the bidet studies found is in is a bidet good for hemorrhoids?, and barrier ointments are compared on the best hemorrhoid ointments.

Creams for an itchy anus
Hydrocortisone
A mild steroid cream is the usual next step if cleaning and drying are not enough. In a small placebo-controlled trial of 1% hydrocortisone for anal itching, 10 people with pruritus ani used 1% hydrocortisone ointment and a placebo for 2 weeks each 6. Hydrocortisone cut itch scores by 68 percent compared with placebo and improved the look of the skin 6. That is a small trial.
Steroid creams are meant for short courses. The NHS says hydrocortisone products for piles and an itchy bottom should not be used for more than 7 days unless a doctor tells you to 7. A 2026 expert review of hemorrhoid treatment says topical steroids should not be used for more than 2 weeks at a time 8. If you need a steroid cream for longer, the cause needs a look. How steroid and numbing creams differ is in anti-itch or numbing cream.
Capsaicin and other prescription options
When itching lasts for months with no cause found, doctors have other options, all used under supervision.
- Capsaicin, the compound that makes chili peppers hot, at a very low strength. In a placebo-controlled crossover trial of capsaicin ointment, 31 of 44 people with long-lasting, unexplained anal itching got relief on capsaicin and not on the placebo 9. Four stopped because of side effects, and people who responded needed to keep applying it to stay itch-free 9.
- Tacrolimus ointment, an immune-calming skin treatment, is listed in reviews for resistant cases 3.
- Methylene blue injections under the skin. A review of 7 studies with 225 people found the itch resolved in about 76 percent after one injection, with no severe complications, though the studies were of poor quality 10.
Itchy anus and hemorrhoids
If you have hemorrhoids, treating them helps the itch. Soft stools mean less wiping and less leakage of mucus. Warm sitz baths soothe the skin, and a cooling witch hazel pad can calm a flare for a few days. Itch from hemorrhoids often feels worst at bedtime, and why hemorrhoids itch more at night explains the routine that helps. Natural options, from witch hazel to coconut oil, are graded in natural remedies for hemorrhoids.

When to see a doctor about an itchy anus
A review of anal itching says a careful history and examination, including a look inside the anus, are needed because there are so many possible causes 3. See a doctor if:
US guidelines recommend colon cancer screening from age 45 for adults at average risk 12. A clinician can check for skin conditions such as eczema or psoriasis, fungal and other infections, fissures and hemorrhoids, and take a small skin sample if anything looks unusual. Not sure whether hemorrhoids are part of the picture? The hemorrhoid symptom checker can help you sort the symptoms first.
Frequently asked questions
Why does my anus itch more at night?
Many people notice itch more at night, partly because there is less to distract them. Scratching in your sleep then keeps the skin irritated. Pinworms also cause itching that is typically worse at night, especially in children. Our guide on night-time hemorrhoid itch covers the bedtime routine.
Can hemorrhoids cause anal itching without pain?
Yes. Internal hemorrhoids often cause little pain, but they can leave mucus or tiny traces of stool on the skin, and that moisture makes the skin itch. External hemorrhoids and skin tags can make cleaning harder, which has the same effect.
What foods cause an itchy anus?
Reviews list food and drink irritants as a cause, but the evidence is weak and the culprits differ from person to person. The ones worth testing are the ones you notice a link with. Cut out one suspect at a time for 2 weeks and see whether the itch changes.
Is Vaseline good for an itchy anus?
A thin layer of a plain barrier ointment such as petroleum jelly or zinc oxide can protect the skin from moisture after you clean and dry it. It does not treat an infection or a skin disease, so if the itch is no better in a couple of weeks, get checked.
Can an itchy anus be a sign of cancer?
Rarely. Anal cancer and some precancerous skin changes can itch. That is why an itch that lasts more than a few weeks, or comes with a lump, a sore, bleeding or skin that looks white, thickened or ulcerated, should be examined.
