Quick answer

Neither has been tested on an ordinary hemorrhoid flare. Witch hazel is a cooling astringent, handy as a pad for itching and burning, with mild effects in skin studies. Aloe has the better hemorrhoid-specific result: it eased pain and healing after hemorrhoid surgery. For a flare, use whichever soothes you for about a week, alongside fiber.

Key takeaways

  • Neither witch hazel nor aloe vera has a trial in people with an ordinary hemorrhoid flare.
  • Aloe cream lowered pain and improved healing in a 49-person trial after hemorrhoid surgery, and ranked first for 24-hour pain in a review of 26 surgery trials.
  • In skin tests, a 10 percent witch hazel lotion cut redness by about 20 percent at 7 hours. In eczema, witch hazel cream did no better than the plain cream base, and weak hydrocortisone did more.
  • Witch hazel pads are better for wiping and cooling. Aloe gel is better for leaving on dry, sore skin.
  • Use either on the outside skin only, for about a week, and stop if it stings or a rash appears. Fiber has far stronger evidence than both.
On this page

Witch hazel and aloe vera are the two plant remedies people reach for first when a hemorrhoid flares. They are often sold side by side and sometimes in the same product, but they do different jobs. Witch hazel is an astringent liquid, usually on a pad, that cools and tightens the skin. Aloe is a gel that soothes and sits on the skin.

The honest answer to “which is better” is that neither has been tested on an ordinary flare. Aloe has a stronger result in hemorrhoid surgery patients, and witch hazel has a few small skin studies. This page lines them up, explains which suits which symptom, and shows how to use both without irritating sore skin. Each has a longer page of its own: does witch hazel work for hemorrhoids? and aloe vera for hemorrhoids.

The American Gastroenterological Association’s 2026 review of hemorrhoid treatment lists astringents such as witch hazel among topical options that can be considered for hemorrhoids, and says there is little data that topical products work 1. That applies to both.

What the witch hazel studies found

The witch hazel research is on skin, not hemorrhoids.

  • In 30 healthy volunteers with a controlled sunburn-type redness, a lotion with 10 percent witch hazel cut redness by about 20 percent at 7 hours and about 27 percent at 48 hours, more than the comparison lotions 2.
  • In 24 adults, a low-dose witch hazel cream reduced redness caused by ultraviolet light at 24 hours, a fourfold higher dose did no better, and 1 percent hydrocortisone looked stronger 4.
  • In a double-blind trial of witch hazel in 72 people with eczema, witch hazel cream did no better than the plain cream base over 14 days, and 0.5 percent hydrocortisone reduced symptoms more 3.

So witch hazel has a mild calming effect on irritated skin, weaker than a low-strength steroid cream.

What the aloe studies found

Aloe has a result in people with hemorrhoids, but only after surgery.

  • In a placebo-controlled trial of aloe cream after hemorrhoid surgery, 49 people used aloe cream or a placebo cream for 4 weeks. The aloe group had less pain in the first 48 hours and at 2 weeks, less pain with bowel movements, better wound healing at 2 weeks, and needed fewer extra pain pills 5.
  • A network analysis of 26 surgery trials ranked aloe cream first for pain at 24 hours and for pain during bowel movements, while calling the overall results inconclusive 6.
  • A Cochrane review of aloe on wounds found the trials were poor quality, and in one trial of open surgical wounds aloe slowed healing 7.

How to use witch hazel and aloe safely

Both are for the outside skin only.

  1. After a bowel movement, rinse with water and pat dry.
  2. For witch hazel, dab with a pad, or fold it and rest it against the area for a few minutes as a cool compress.
  3. For aloe, smooth on a thin layer of plain gel once the skin is dry.
  4. Using both? Witch hazel first, let it dry, then aloe.
  5. Stop if the skin stings, gets very dry, or a rash appears. NIH lists burning, itching, rash and eczema as occasional reactions to aloe gel 8.
  6. Give it about a week. NIDDK says hemorrhoid products are usually used for a week, and to follow up if symptoms remain or you get dry skin or a rash 9.

Do not drink aloe for hemorrhoids. The latex from the outer leaf is a harsh laxative that the FDA had removed from over-the-counter laxatives in 2002 8. The aloe page explains why diarrhea makes hemorrhoids worse.

The best-rated hemorrhoid product we found in any category. The pads are soft, cooling and gentle enough for everyday use.

What recent buyers say

Tens of thousands of reviews, and the theme never changes: soothing, quick relief, and near-universal praise from people recovering after childbirth. Many fold a pad and leave it in place as a compress for 10 to 15 minutes. The one common gripe is that the pads stick together and can be fiddly to separate.

A sitz bath basin on a toilet seat next to a kettle and towel, with a bottle of witch hazel and a small aloe plant on a shelf above
Either plant is a comfort step. A warm soak and daily fiber do more for the flare itself.

What works better than both

Witch hazel and aloe are comfort steps. The steps with stronger evidence for the hemorrhoid itself are:

  • Fiber and water every day. In seven trials, fiber cut the risk of bleeding by half and of lingering symptoms by 47 percent 10.
  • Warm sitz baths for spasm pain, and a wrapped cold pack for swelling.
  • Short toilet sits with no straining.

All of these, and 21 natural remedies in total, are graded in natural remedies for hemorrhoids.

Frequently asked questions

Can I use witch hazel and aloe vera together on hemorrhoids?

Yes, for most people. A common routine is to clean with water, dab with a witch hazel pad, let the skin dry, then smooth on a thin layer of plain aloe gel. If your skin stings, drop one of them and see which caused it.

Which is better for hemorrhoid itching, witch hazel or aloe?

Neither has an itching trial in hemorrhoids. Witch hazel pads give a quick cooling feel that many people find helps itching. If the itch is strong, a short course of 1 percent hydrocortisone has better evidence than either plant.

Is witch hazel or aloe better after hemorrhoid surgery?

Aloe has the surgery evidence: an aloe cream reduced pain and sped healing after hemorrhoidectomy in a placebo-controlled trial. Ask your surgeon before putting anything on a wound, and follow their aftercare plan.

Does witch hazel dry out the skin?

It can. Witch hazel is an astringent, which gives a tight, dry feeling. NIDDK advises following up if you get dry skin or a rash around the anus while using a hemorrhoid product. Aloe gel or a barrier ointment can help if the skin feels too dry.

Is witch hazel or aloe safe during pregnancy?

Neither has been tested for hemorrhoids in pregnancy. NIH warns against taking aloe by mouth in pregnancy, which is a different use from gel on the skin. Check any product with your obstetric provider.
