Quick answer

Fiber has the best evidence of any home remedy for hemorrhoids. Water helps that fiber. Warm soaks and a wrapped ice pack are reasonable for comfort, with thinner trial data. Witch hazel has little trial support. Skip apple cider vinegar and undiluted tea tree oil. Aloe rests on one small study after surgery.

Key takeaways

  • In seven randomized trials, fiber lowered the chance that hemorrhoid symptoms would persist by 47 percent and lowered the chance of bleeding by 50 percent.
  • Adults with constipation who were already eating about 25 grams of fiber had more frequent stools when they drank about 2 liters of water a day than when they drank about 1 liter.
  • Sitz baths are widely suggested, and the trial evidence is thin. Warm water at 40°C lowered resting anal pressure in a lab study. Ice-cold water did not.
  • A review of topical hemorrhoid treatments found little data that witch hazel works. One small trial of aloe cream was done after hemorrhoid surgery, not on an ordinary flare.
  • No clinical trial has tested apple cider vinegar or coconut oil for hemorrhoids. Pure tea tree oil is a known cause of allergic skin rash, and the only hemorrhoid gel trial mixed it with two other ingredients.

Home remedies for hemorrhoids are not all in the same category. Fiber has randomized trials behind it. Warm soaks and a wrapped ice pack are reasonable comfort steps with thinner proof. Witch hazel is a drugstore habit with little trial data. Apple cider vinegar, coconut oil, and pure tea tree oil do not belong in the “works” pile.

The timed version of the useful steps, hour by hour and through day 14, is in how to get rid of hemorrhoids fast. Office treatments and surgery, for when home care is not enough, are in hemorrhoid treatment. This page grades the remedies people actually try in the kitchen and the bathroom. Flavonoid pills, herbal supplements and 21 natural options in all are graded side by side in natural remedies for hemorrhoids.

A grade here means what the studies can support, not how often a remedy shows up in a video. “Best evidence” means more than one trial in people with hemorrhoids. “Thin evidence” means clinicians suggest it, or a lab study explains why it might ease pain, and a proper trial is missing or weak. “Skip” means there is no hemorrhoid trial, or the remedy can hurt skin.

Fiber is the home remedy with the strongest trials

The NIDDK page on eating for hemorrhoids says high-fiber foods make stools softer and easier to pass, and that water and other liquids help that fiber do its job 2. The amount NIDDK cites is 14 grams of fiber per 1,000 calories. On a 2,000-calorie day, that is 28 grams 2. A medium pear with the skin is about 5.5 grams. A cup of raspberries is about 8 grams. A half cup of cooked navy beans is about 9.6 grams 2. Only about 5 percent of Americans meet the recommendation 4.

An adult at a kitchen table eating a bowl of oatmeal topped with pear slices, with a glass of water and a small bowl of beans beside the plate
A medium pear with the skin has about 5.5 grams of fiber. A half cup of cooked navy beans has about 9.6 grams.

A 2006 meta-analysis of seven fiber trials randomized 378 people with symptomatic hemorrhoids to fiber or a non-fiber control 1. The chance that symptoms would persist fell by 47 percent with fiber (relative risk 0.53). The chance of bleeding fell by 50 percent (relative risk 0.50). Pain, itching, and prolapse might improve too, but those results were too uncertain to be sure. Studies that checked more than once, usually at about 6 weeks and at 3 months, kept showing the same pattern 1.

Psyllium is the fiber several of those hemorrhoid trials used, and it is the one NIDDK names 3. In a constipation trial, psyllium at 5.1 grams twice a day raised stool water more than docusate sodium at 100 mg twice a day 7. By treatment week 2, people on psyllium averaged 3.5 bowel movements a week, and people on docusate averaged 2.9 7. Another constipation trial gave psyllium at 12 grams a day, two kiwifruits a day, or 100 grams of prunes a day for 4 weeks. Straining improved with all three by weeks 3 and 4 6. Gas was most common with psyllium 6. Start with part of a dose and build over a few days.

Food lists, and how to add fiber without a miserable week of bloating, are in the hemorrhoid diet. If you want to see the gap on a normal day of eating, use the calculator below or the full fiber calculator. Powders, capsules, and gummies are compared on the best fiber supplements for hemorrhoids.

You eat about 0 g a day from the foods above.

Add a few foods to see your gap.

Targets are the Adequate Intakes set by the National Academies. Food values are rounded averages from USDA FoodData Central. Raise fiber over two or three weeks and drink more water as you go, or you may feel bloated.

Water helps the fiber you just added

Drinking more water, by itself, is not a hemorrhoid trial. It does change constipation when fiber is already in the diet. In 117 adults with chronic constipation, both groups ate about 25 grams of fiber a day for two months 5. One group drank what they liked and averaged 1.1 liters of fluid. The other was told to drink 2 liters of mineral water and averaged 2.1 liters. Stool frequency rose in both groups. The rise was greater in the group that drank more, and they used fewer laxatives 5.

NIDDK’s practical version is to drink water or other nonalcoholic liquids each day, in the amount your own clinician recommends for your health, activity, and climate 23. A full glass with each fiber dose is a simple way to copy the pattern of that trial without turning water into a contest.

Sitz baths: useful, and the trials are thin

A sitz bath is a soak of the anal area in a few inches of warm water. NIDDK says to do this several times a day for pain 3. The NHS says a warm bath eases itching and pain 12. The AGA expert review says sitz baths are often advised and that the scientific data are limited 11. A 2005 review searched the literature and found the same gap: little trial evidence, a note that cold sitz baths reduced swelling more than warm ones in the reports they found, and a preference among patients for warm water 8.

The muscle effect is clearer than the “does the hemorrhoid shrink” effect. Immersion in 40°C water lowered resting anal pressure. Water at 5°C and at room temperature did not 9. After 5 minutes at 40°C, the lower pressure lasted about 15 minutes in volunteers without anorectal problems and about 30 minutes in patients who had them 9. A study of the warm-water bath tied the pain relief to relaxation of the internal anal sphincter 10. So the soak is a comfort tool you can repeat. It is not the remedy that cut bleeding in half. Temperature, timing, and what not to add are in how to take a sitz bath. Basins are compared on the best sitz baths.

Cold packs for swelling

The NHS says to use an ice pack wrapped in a towel for discomfort 12. It does not name a minute count.

The clearest ice trial was done after surgery, not during an ordinary flare. It compared ice packing with warm sitz baths in 166 people after hemorrhoidectomy 13. Ice packing gave lower pain scores within 16 hours, and at 24 hours 76.8 percent of that group had no or mild swelling, compared with 56.0 percent after warm sitz baths. They also used less morphine that first day 13. Use a wrapped pack for throbbing. Keep the warm soaks for the tight muscle. Which to reach for on a given day is in ice or heat for hemorrhoids. Packs made for this are on the best ice packs for hemorrhoids.

Witch hazel, without repeating the whole debate

Witch hazel is an astringent, a plant liquid that tightens the feeling of wet, itchy skin. The AGA review lists astringents, and names witch hazel, among topical treatments that can be considered, and it says there is little data that those topicals work 11. That is the grade: a common pad, a thin evidence file.

Hydrocortisone is a different product. The NHS instructions for hydrocortisone for piles say not to use it for more than 7 days 19. Who finds witch hazel pads soothing is covered in does witch hazel work for hemorrhoids?.

Aloe: one small trial, after surgery

Aloe vera shows up in home lists because the gel feels cool. The hemorrhoid trial did not test a leaf from a windowsill. It tested a prepared aloe cream on the surgical site after open hemorrhoidectomy 14. Forty-nine patients used the aloe cream or a placebo cream three times a day for 4 weeks. The aloe group had less pain at 12, 24, and 48 hours and at 2 weeks, less pain with bowel movements at 24 and 48 hours, better wound healing at 2 weeks, and they used fewer extra pain pills 14.

That is a real result, and it is a narrow one: 24 people on aloe, right after an operation, using a cream made for the study. It does not show that squeezing a houseplant onto a flare will do the same thing. If you are recovering from hemorrhoid surgery, ask your surgeon before you add any cream. How to use aloe gel safely, and why not to drink it, is in aloe vera for hemorrhoids. If you are treating a flare at home, fiber, water, soaks, and a wrapped cold pack are the remedies with a broader basis.

Coconut oil, apple cider vinegar, and tea tree oil

These three travel together online. They should not travel together in your bathroom.

Coconut oil: a search of PubMed for coconut oil used on hemorrhoids did not find a clinical trial. There is no result to quote for pain, bleeding, or healing.

Apple cider vinegar: a PubMed search for apple cider vinegar and hemorrhoids returned no clinical trials. Vinegar is an acid. In a published case, vinegar put on the skin of a newborn to bring down a fever caused a chemical burn 17. That baby was not being treated for hemorrhoids. The case is still a reason to keep vinegar off skin that is already sore. Garlic sits in the same viral group. The checks on vinegar, garlic, coconut oil, and tea tree oil are in apple cider vinegar and other viral hemorrhoid remedies.

Tea tree oil is the one of the three with both a hemorrhoid trial and a harm file, and they are not about the same product. The trial tested a gel that contained hyaluronic acid, tea tree oil, and methyl-sulfonyl-methane, against placebo, in 36 people with grade 1 to 3 hemorrhoids for 14 days 16. The gel group had lower scores for anal pain, pain with bowel movements, bleeding, itching, and irritation. Tolerability was described as good 16. Thirty-six people and three ingredients in one gel cannot tell you that tea tree oil was the active part.

Pure tea tree oil is a different story. A review of contact allergy found that tea tree oil has caused more published allergic reactions than other essential oils since the first cases in 1991 15. In routine patch testing, positive reactions ranged from 0.1 percent to 3.5 percent. Nearly 100 allergic patients have been written up in case reports. Most reactions came from putting pure oil on the skin, not from a cosmetic that contained a little of it. Fresh oil is a weak to moderate sensitizer, and oxidized oil is more allergenic 15. Do not drip essential oil onto a hemorrhoid. The trial, the allergy data and safer options are in tea tree oil for hemorrhoids.

Fiber and water belong in the plan. Vinegar and undiluted tea tree oil do not belong on sore skin.

When a home remedy is the wrong tool

Home care has a clock. NIDDK and MedlinePlus say to see a clinician if you are no better after 1 week 318. MedlinePlus also says rectal bleeding needs a check because hemorrhoids are only one cause. Crohn’s disease, ulcerative colitis, colorectal cancer, and anal cancer can bleed too 18. The US Preventive Services Task Force recommendation is to start colon cancer screening at age 45 for adults at average risk 20.

Pregnancy is mostly a home-care problem, with a shorter menu. A review for family doctors suggests fiber, stool softeners, more liquid, and toilet habits, and says most symptoms ease after birth 21. Common topical products have not been formally tested in pregnancy 21. Check any cream, including a “natural” one, with your obstetric provider. The full plan is in hemorrhoids during pregnancy.

If the week of fiber, water, soaks, and a wrapped pack has not changed anything, the next page is not another kitchen experiment. It is over-the-counter hemorrhoid treatment for a short course of cream, or when to see a doctor for hemorrhoids if bleeding or a prolapse is the reason you are still uncomfortable. Keeping the flare from coming back, once this one settles, is the job of how to prevent hemorrhoids.

Frequently asked questions

How much fiber should I eat if I have hemorrhoids?

The figure NIDDK cites is 14 grams of fiber for every 1,000 calories, which is 28 grams a day on a 2,000-calorie diet. Only about 5 percent of Americans reach the recommendation. A psyllium supplement is a practical way to close the gap. Raise the dose over several days and drink a full glass of water with it, because a sudden jump in fiber causes gas.

Can I take a sitz bath every day?

Yes. NIDDK suggests a warm sitz bath several times a day during a flare. The trial evidence that this shrinks hemorrhoids is limited. What is measured is muscle relaxation: a 5 minute soak in 40°C water lowered resting anal pressure for about 15 minutes in healthy volunteers and about 30 minutes in people with anorectal problems. Use water that feels warm on your wrist.

Is witch hazel the same as a medicated hemorrhoid cream?

No. Witch hazel is an astringent, a plant liquid used for a cool, dry feeling on skin. Hydrocortisone is a steroid and has a 7 day limit on the NHS instructions for piles. A numbing cream is a third type. Witch hazel can sit alongside fiber. It does not replace it, and the trials behind it are much thinner than the trials behind fiber.

Are home remedies enough if a hemorrhoid keeps slipping out?

Fiber and softer stools are still worth doing, because straining makes prolapse harder to live with. A hemorrhoid that slips out and has to be pushed back, or one that stays out, often needs an office treatment or surgery. Home remedies are the first step, not the last one, when prolapse is the main problem.

How long should I try home remedies before I call a doctor?

About a week. NIDDK and MedlinePlus both say to contact a clinician if symptoms are no better after 1 week of home treatment. Go sooner for heavy bleeding, severe pain, fever, or a new lump that appeared overnight and makes sitting unbearable. New rectal bleeding at age 45 or older should be checked even if you think it is a hemorrhoid.
