Quick answer

Use a suppository when swelling and discomfort are inside, especially at night. Preparation H suppositories are the usual first try. Calmol 4 is the gentler option if you do not need numbing. If you would rather not insert anything, a wrapped cold pack on the outside is the drug-free alternative.

Key takeaways

  • Suppositories are for internal hemorrhoids. Creams and most ointments stay on the outside skin.
  • The NHS says hydrocortisone suppositories are for internal piles, usually morning, at night, and after a bowel movement, and not for more than 7 days.
  • An older study of 89 people found ointment and suppository versions of two prescription products both eased second-degree hemorrhoids, with no placebo group to compare.
  • A cold pack is not inserted. After hemorrhoid surgery, ice packing eased pain more than warm sitz baths in the first 16 hours in one trial.
  • Fiber still has the best trial evidence for bleeding. A suppository that has not helped in a week needs a doctor, not a longer course.
On this page

A hemorrhoid suppository is for swelling and discomfort inside, especially at night. Preparation H suppositories are the ones most buyers try first. Calmol 4 is the gentler option when you do not want a numbing product and you can keep the box cool. If you do not want to insert anything, the Hemorrwedge cold pack rests on the outside and is the drug-free pick on this page.

Suppositories are the wrong first buy when the sore spot is on the skin you can see. That is a cream or ointment problem. Internal hemorrhoids, the ones inside the rectum, more often announce themselves with bleeding than with sharp pain. This page explains when a suppository makes sense, how to use one, and how strong the evidence is. Start with internal hemorrhoids if you are not sure which type you have, and with the best hemorrhoid products if you want the wider set.

The NHS instructions for hydrocortisone suppositories say they are for piles on the inside. Creams and ointments are for the outside 2.

Our picks compared

Two of these are suppositories. The third is a cold pack for people who want relief without inserting a medicine.

Reviews of our picks

Buyer comments cluster around nighttime use for Preparation H, a gentler feel for Calmol, and short cold sessions for the pack. The complaints are mostly practical: foil, heat, and seams.

Best overall
Preparation H (Suppository)

Preparation H Suppositories

Best for: Swelling and discomfort inside, especially overnight

Inside use12 per box

These are the suppositories most people try first for swelling and discomfort inside, especially at night. Buyers say relief comes on quickly and that bedtime is when they help most. The foil can be hard to open without scissors, and some arrive cracked or crumbly. If the soreness is only on the outside skin, use a cream.

What recent buyers say

Buyers this summer say they work quickly and help most when used at night. One pointed out that a store brand with the same formula costs less. The recurring complaints are practical: the foil can be hard to open without scissors, and some suppositories arrive cracked or crumbling. Price comes up a lot for people who use them nightly.

What buyers like

  • Quick relief of discomfort inside
  • Handy to use overnight
  • Easy to insert once unwrapped

Common complaints

  • Foil wrapper is hard to open
  • Some arrive cracked or crumbled
  • Adds up if you use one every night

Skip it if your symptoms are only on the outside. A cream or ointment reaches that skin better.

Best gentle option
Calmol 4 (Suppository)

Calmol 4 Hemorrhoidal Suppositories

Best for: Gentle, regular use when you do not need numbing

Inside use24 per boxNo numbing agent

Calmol 4 is the gentler suppository for regular use when you do not need numbing. Buyers describe soothing relief over a couple of days, and the box holds more than the Preparation H box. Heat is a real problem: boxes can arrive melted in summer, and some suppositories crumble when unwrapped. Skip it if the package will sit in a hot mailbox.

What recent buyers say

Many of this summer's reviews just say it works, and older ones describe relief within a couple of days. Heat is the enemy. Some boxes arrive melted in summer, and one buyer in August could not get a refund. Others find the suppositories crumble or feel dry.

What buyers like

  • Soothing relief that lasts
  • Inserts easily
  • Box of 24 lasts a while

Common complaints

  • Can arrive melted in hot weather
  • Some crumble when unwrapped

Skip it if you live somewhere hot and cannot bring the package inside quickly.

Hemorrwedge (Cold pack)

Hemorrwedge Hemorrhoid Ice Pack (2 pack)

Best for: Swelling and throbbing when you want relief without medicine

ReusableTwo packsStorage case

This cold pack is the drug-free option when swelling and throbbing are the problem and you do not want to insert anything. It rests on the outside. Buyers describe real relief, and the two-pack means one can freeze while you use the other. The cold lasts about 15 to 20 minutes. Wrap it, because bare gel from the freezer is too cold, and the seams can scratch or split over time.

What recent buyers say

Most reviewers who mention results call it a lifesaver for pain and swelling. The cold lasts about 15 to 20 minutes, which matches the usual advice anyway. The common complaints are hard seams that can scratch and packs that split after a while. Wrap it in a thin cloth, since some find it too cold straight from the freezer.

What buyers like

  • Real relief from swelling and pain
  • One pack freezes while you use the other
  • Case keeps it separate from food

Common complaints

  • Hard seams can dig in
  • Some split at the seam over time
  • Too cold without a cloth

Skip it if you want long cold sessions. Each pack stays cold for about 15 to 20 minutes.

How we chose these hemorrhoid suppositories

The suppository aisle is smaller than the cream aisle. We kept the two with strong overall ratings from a large number of buyers and a clear split in the reviews. Preparation H is the one people already know, and recent comments say it helps most at night once the wrapper is open. The foil is stiff, some pieces arrive cracked, and a nightly habit uses them up. Calmol’s reviews say it soothes over a couple of days and the larger box lasts. The failure mode is heat: summer deliveries can arrive melted, and crumbled pieces are common enough to mention.

Hemorrwedge sits on this list as a cold pack, for people who have internal swelling and also want something drug-free for throbbing on the outside. Reviews praise the relief and the two-pack system. They also mention hard seams, packs that split later, and cold that is too intense without a cloth. Buyers say the cold lasts about 15 to 20 minutes. We did not pick by price. A product with a melted-in-transit problem stays on the list with that warning attached. No brand pays for a slot. Details are on the affiliate disclosure.

What the research says

Conservative care for hemorrhoids has long included suppositories. A widely cited review lists lifestyle changes, fiber, suppository-delivered anti-inflammatory drugs, and vein-toning medicines as the usual non-surgical start, and names bleeding with bowel movements as the most common symptom 3. NIDDK says creams, ointments, or suppositories may relieve mild pain, swelling, and itching, including of external hemorrhoids, for about a week 1. MedlinePlus describes internal hemorrhoids as the ones that bleed during bowel movements, and external ones as the ones that make cleaning difficult and hurt if a clot forms 11.

The best direct comparison of an ointment and a suppository is old, open-label, and not a test of the products on this page. In 1988, 89 people in general practice with second-degree hemorrhoids were assigned to one of two prescription steroid-and-antibiotic products, each in an ointment and a suppository. All four groups improved. Symptom control showed up from week 2 and was above 90 percent by week 3. Suppositories gave slightly more relief of pain and itching early on. Bleeding fell similarly with ointment and suppository. No side effects were blamed on treatment. There was no placebo arm, so the study cannot tell you how much was the medicine and how much was time 4.

A 2019 review of hemorrhoid research is more cautious. Apart from fiber, medical therapies have not been formally studied, and fiber results themselves have been inconsistent across papers 6. Family physicians still start with fiber, fluids, topical products, and sitz baths 7. Read what the studies say about over-the-counter treatment before you treat a suppository as a cure for bleeding.

Fiber is the part with a meta-analysis. Seven trials, 378 people: the risk of bleeding was about half in the fiber group (relative risk 0.50), and the risk that symptoms would not improve fell by 47 percent 17. In pregnancy, a large comparison gave everyone a glycerin suppository as a lubricant before bowel movements, plus daily psyllium. The groups differed in sitz baths versus a cream, not in whether a suppository was used. Healing was common in both 5. A Cochrane review found diet changes and local treatments in pregnancy have not been well tested in randomized trials 19.

Hydrocortisone suppositories have a clear limit even though our two boxes are not automatically that drug. The NHS how-to page says not to use hydrocortisone treatments for piles for more than 7 days, and to see a doctor if you are no better or symptoms return quickly 2. The same page says they can be used morning, at night, and after a bowel movement. Follow the leaflet in your box if the schedule differs.

The cold pack has a different evidence trail. In a lab study, cold water did not lower resting pressure in the anal canal, while warm water did 9. Warm water also lowered pressure in people with hemorrhoids in a separate bath study, with the effect lasting after they got out 18. So a cold pack is not “doing what a sitz bath does.” It is for the swelling and throbbing people feel on the outside. The NHS piles page suggests an ice pack wrapped in a towel for discomfort 10. After hemorrhoid surgery, a 2025 trial of 166 people found ice packing lowered pain more than warm sitz baths in the first 16 hours, with less swelling at 24 hours and less morphine in the first day 8. That is surgical pain. It supports a short cold session. It does not mean ice replaces a suppository for internal swelling. Ice or heat for hemorrhoids and the ice pack guide go further.

Pregnancy: the NHS page on hydrocortisone suppositories in pregnancy says they can be used then, because little of the medicine reaches the blood, and for a short time while breastfeeding if you wash your hands first 15. A study of 204 women who used a hydrocortisone-and-numbing foam in the third trimester found no rise in prematurity or birth complications 14. Everyday products have not each been tested in pregnancy 13. Ask your obstetric provider before a medicated suppository. Hemorrhoids during pregnancy covers safer first steps, including fiber.

Outside burning belongs on the best hemorrhoid creams. If you want an ointment that is labeled for both inside and outside, that comparison is on hemorrhoid ointments. Bleeding with bowel movements still needs fiber even if a suppository makes the evening more comfortable. The fiber guide and bleeding hemorrhoids are the pages for that symptom.

How to use them safely

A woman in pajamas lies on her side in bed, with a small box, a glass of water, and a folded towel on the nightstand.
Stay on your side for about 15 minutes so a suppository can melt before you stand up.

These steps are the ones the NHS gives for hydrocortisone suppositories. Use them as the pattern, and follow your own leaflet if it disagrees 2.

  1. Go to the toilet first if you need to. A suppository that goes in just before a bowel movement often comes straight back out.
  2. Wash your hands. Clean around the anus with mild soap and water, rinse, and pat dry.
  3. Unwrap the suppository. If the foil will not tear, use scissors so you do not crush it. Throw away a piece that is cracked, crumbly, or melted into the wrapper.
  4. Lie on your side with one knee bent, or stand with one foot on a chair. The pointed end goes in first, about 2 to 3 centimeters (about an inch).
  5. Stay still for about 15 minutes so it can melt.
  6. Wash your hands again.
  7. For a hydrocortisone suppository, stop at 7 days 2. NIDDK’s usual limit for over-the-counter hemorrhoid products is about a week. Call if symptoms are the same, or if you get a rash or very dry skin 1.
  8. Store Calmol somewhere cool. Do not leave either box in a hot car.
A man reclines on a sofa with a blue cold pack resting on a folded cloth beside him.
Keep a cold pack on a thin cloth for about 15 to 20 minutes. Bare gel from the freezer is colder than most people can stand.

For the Hemorrwedge pack, freeze one while you use the other. Wrap it or set it on a thin cloth. Buyers get about 15 to 20 minutes of cold. Hard seams can dig in, so shift it if a edge pokes. Some packs split at the seam after months of use. Do not fall asleep with a bare pack against the skin. The NHS version of this advice is an ice pack wrapped in a towel 10.

While breastfeeding, keep any hydrocortisone suppository to a short course and wash your hands before you hold the baby 15. If feeding changes, call your clinician.

When to see a doctor instead

A suppository will not explain dark blood, a change in bowel habits, or weight loss. The American Cancer Society’s page on colorectal cancer symptoms lists blood that makes stool look dark brown or black, a bowel change lasting more than a few days, belly pain, weakness, and unintended weight loss 16. The US Preventive Services Task Force recommends screening from age 45 for adults at average risk 12.

Get care right away for severe anal pain and rectal bleeding, particularly with belly discomfort, diarrhea, or fever 1. Bright red bleeding can still be an internal hemorrhoid, and it still deserves a look if it is new. When to see a doctor for hemorrhoids describes the visit. If a week of a suppository plus fiber has not changed the bleeding, the next step is an exam, and sometimes banding, which the treatment overview explains.

Frequently asked questions

Do suppositories work for external hemorrhoids?

They are a poor match. External hemorrhoids sit under the skin around the anus, where a cream or ointment can reach them. A suppository melts inside the rectum. NIDDK says suppositories may still ease mild external symptoms for some people, but if you can see and touch the sore spot, start with a cream.

How long do you leave a hemorrhoid suppository in?

The NHS tells people using hydrocortisone suppositories to stay sitting or lying down for about 15 minutes so the suppository can melt. You do not pull it back out. If it comes out right away, you were probably not still long enough, or you needed the toilet first.

Can I use a suppository every night?

Hydrocortisone suppositories for piles are limited to 7 days unless a doctor says otherwise. Nightly use for weeks is longer than that window, and buyers of Preparation H mention that a nightly habit uses the box up fast. If you still need one every night after a week, get checked.

What if the suppositories arrive melted or broken?

Heat softens them, and Calmol buyers report melted boxes in summer. Cracked or crumbly suppositories are a recurring complaint with both brands here. A broken one will not stay in place. Keep the box in a cool room, out of a hot car or mailbox, and contact the seller if the whole box arrived ruined.

Is a cold pack as useful as a suppository?

They do different jobs. A suppository treats the inside. A cold pack rests on the outside and is for throbbing and swelling you can feel, without medicine. After surgery, one trial found ice helped pain more than a warm soak in the first 16 hours. For an ordinary flare, many people use the pack for 15 to 20 minutes at a time.
