Quick answer

After a vaginal birth, start with a rinse bottle and cold on the swelling, then add a warm sitz bath if you can soak. Hemorrhoids cluster in the third trimester and the first weeks after delivery, and many settle once stools are soft. Fiber and water do that job. Check any medicated cream with your obstetric provider before you use it, in pregnancy and while breastfeeding.

Key takeaways

  • In one cohort of 280 pregnant women, 40.7% were diagnosed with hemorrhoids by one month after birth, mostly in the third trimester or just after delivery.
  • Constipation was the strongest repeating risk factor in two pregnancy cohorts. A small trial found fiber added about two bowel movements a week.
  • The NHS suggests a cloth wrung out in iced water, soft stools, and no straining. ACOG also lists ice packs and a warm bath for hemorrhoid symptoms.
  • A pregnancy study recorded healing in every woman on a warm sitz-bath routine, versus 84.8% on a cream, and both groups also took psyllium. It was not a blinded trial.
  • One third-trimester study of a hydrocortisone-pramoxine foam found no increase in birth complications. That does not cover every cream, and breastfeeding was not studied there.
On this page

Hemorrhoids after birth are common, and the useful first steps are simple: rinse instead of wipe, cool the swelling, soak in warm water if you can, and keep stools soft. For the first days at home we would put a Frida Mom peri bottle and a cold pad within reach, then add the Fivona sitz bath once getting to the bathroom feels possible.

In a cohort of 280 pregnant women followed to one month after delivery, 40.7% were diagnosed with hemorrhoids 1. Most of those problems showed up late in pregnancy or just after the birth, and a Canadian review notes that for most women the symptoms ease soon after delivery 3. Our postpartum hemorrhoid guide walks through that timeline. This page is the product side: which of the seven items matches itching, swelling, or sitting, and which creams to clear with your obstetric provider first.

Our picks compared

These seven do different jobs. A rinse bottle does not replace a soak, and a cushion does not replace fiber.

The line under each pick says who it is for. The sections below explain the research, the order to use them in, and the safety limits in pregnancy and while breastfeeding.

Best sitz bath after birth
Fivona (Sitz bath)

Fivona Foldable Sitz Bath

Best for: Most people, especially after childbirth or surgery

Fits most toiletsFolds flatDrain slots

This is the basin we would set up after a vaginal birth. It sits on the toilet, folds flat for a cabinet, and drains into the bowl, which recent buyers prefer to climbing into a tub. People with wider hips sometimes find the seat narrow. If the fit is tight, our sitz bath roundup includes a roomier basin.

What recent buyers say

August 2026 reviews praise how steady it feels on the toilet and how easily it folds into a cabinet. One reviewer used it after hemorrhoid surgery and found it far easier than getting in and out of a tub. The only real split is size, and people with wider hips sometimes find it narrow.

What buyers like

  • Stays steady on the toilet
  • Folds flat for storage
  • Drains cleanly into the bowl

Common complaints

  • Narrow for wider hips

Skip it if you have wide hips. The Pochik has a roomier seat.

Best rinse bottle
Frida Mom (Spray or rinse)

Frida Mom Upside Down Peri Bottle

Best for: Rinsing instead of wiping, at home or on the go

Angled nozzle10 ozTravel pouch

An angled squeeze bottle for rinsing with warm water so you are not wiping sore skin. New parents and people healing after procedures both describe it as the item they reach for every bathroom trip. It holds 10 ounces, so you may refill it, and some buyers report leaks. The angled neck is the feature buyers mention most.

What recent buyers say

This year's reviews come from new mothers and from people healing after procedures, and both groups call it indispensable. The angle is the feature everyone mentions. Some report leaks, one found it noisy, and others wish it held more water.

What buyers like

  • Angled neck reaches easily
  • Gentle, easy-to-control spray
  • Packs into a bag or pocket

Common complaints

  • Can leak
  • Holds only 10 ounces

Best cold pads
Frida Mom (Pads)

Frida Mom 2-in-1 Ice Maxi Pads

Best for: The first days after a vaginal birth

Instant coldAbsorbent padNo freezer needed

A maxi pad with a cold pack you start by squeezing, so the first days do not depend on a freezer. Buyers use them in the first two weeks and say they absorb like hospital pads. They get bulky once the pack is activated, and the cold lasts about 20 minutes. They fit those early days. After that, a reusable pack is the one you can keep using.

What recent buyers say

Many call them life-saving in the first two weeks and say they absorb as well as hospital pads. Opinions split on how cold they get and whether they are worth the money. A common complaint is bulk, especially once the cold pack is activated.

What buyers like

  • Cold relief without a freezer
  • Absorbs like a hospital pad

Common complaints

  • Bulky
  • Cold does not last long
  • Expensive per pad

Best hot and cold pad
Lansinoh (Cold pack)

Lansinoh Hot and Cold Postpartum Pads

Best for: Swelling and hemorrhoid pain in the weeks after birth

Hot or coldReusableFabric sleeves

Slim gel pads you freeze or warm, then wear inside a fabric sleeve. Recent buyers say they feel more comfortable than disposable ice pads, and the sleeves wash. The cold fades in roughly 15 to 30 minutes, and a few people find the edges hard. This is the pack for swelling that is still there after the first week.

What recent buyers say

Mothers writing in 2026 say these beat disposable ice pads, and one bought a second set for her next baby. They are comfortable and easy to clean. The complaint is time, since they stay cold for roughly 15 to 30 minutes, and a few found the edges hard.

What buyers like

  • More comfortable than disposable ice pads
  • Reusable and washable
  • Can be used warm or cold

Common complaints

  • Cold does not last long
  • Edges can feel hard

Best cooling pads
TUCKS (Pads)

TUCKS Medicated Cooling Pads, 100 count

Best for: Itching, burning and postpartum soreness

Cooling100 padsDye-free

Soft cooling pads for itching, burning, and the sore weeks after birth. Buyers often fold one and leave it in place for 10 to 15 minutes, and people recovering from childbirth are a large share of the praise. The pads stick together and can be fiddly to separate. They are a comfort habit. We do not have a trial that tested this pad as a hemorrhoid treatment.

What recent buyers say

Tens of thousands of reviews, and the theme never changes: soothing, quick relief, and near-universal praise from people recovering after childbirth. Many fold a pad and leave it in place as a compress for 10 to 15 minutes. The one common gripe is that the pads stick together and can be fiddly to separate.

What buyers like

  • Instant cooling relief
  • Gentle on sensitive skin
  • 100-count tub is good value

Common complaints

  • Pads stick together
  • Can be messy to handle

Best non-medicated balm
Motherlove (Ointment)

Motherlove Mom's Bottom Balm

Best for: Pregnancy and postpartum soreness when you want a non-medicated option

Herbal balmPregnancy and postpartum1 oz jar

A non-medicated balm for pregnancy and the weeks after birth when you want something that does not sting. Recent notes from new mothers describe it as gentle, and a small amount goes a long way. It is greasy and can stain fabric. It will not numb severe pain. If you need a medicated cream, ask your obstetric provider before you buy one.

What recent buyers say

Recent reviews from new mothers describe it as gentle and soothing, and several say a little goes a long way. Plenty of people outside the target audience like it too. On the downside it is greasy, a few found it stained, and some felt the small jar was poor value. One reviewer received a jar with a broken seal.

What buyers like

  • Gentle, does not sting
  • No odd smell
  • A small amount goes far

Common complaints

  • Greasy
  • Can stain fabric
  • Small jar for the price

Skip it if you need strong pain relief. This is a comfort product, not a numbing one.

Best cushion for sitting
Kieba (Cushion)

Kieba Donut Cushion, 18 inch

Best for: Short-term relief from a sore, swollen spot

Ring shapeWashable coverNon-slip base

A ring cushion for short periods of sitting when a swollen spot makes a chair hard to bear. People who sit for work say it gets them through a spell, and the cover comes off to wash. A sizeable share of buyers find the foam too firm, and some expected a larger ring. Use it for feeds and short sits. All-day sitting for weeks needs a different cushion.

What recent buyers say

Truck drivers, students and desk workers all say it made a real difference. A September reviewer had tried a memory foam ring from a pharmacy and found this one far more comfortable. Firmness divides people: most find the balance right, but a sizeable minority say it is too hard. One recent buyer thought it was small.

What buyers like

  • Comfortable for hours of sitting
  • Holds its shape
  • Cover comes off for washing

Common complaints

  • Too firm for some people
  • Smaller than some expect

Skip it if you will be sitting most of the day for weeks. A contoured seat cushion that supports your thighs may suit you better.

How we chose these postpartum hemorrhoid products

We looked for products with strong overall ratings from a large number of buyers, then read recent reviews from new parents and from people using the same items for hemorrhoids. The comments that repeated were practical: the angled bottle reaches, disposable ice pads are bulky, reusable gel pads feel slimmer, cooling pads stick together, the sitz basin has to sit steady on the toilet, and a ring cushion is too firm for some people.

The Motherlove balm and the Lansinoh pads have fewer reviews than the peri bottle or the cooling pads. We kept them because the recent notes agree. The balm is described as gentle and greasy. The gel pads are described as washable and short-lived once they are cold. Research decided what we will not claim. Fiber has trial evidence for bleeding. A specific balm jar, a donut cushion, and a cooling pad do not. They are here as comfort tools that match what guidelines already suggest: soft stools, cold, warm water, and less wiping 7813.

What the research says

Two cohorts tell you when this shows up. Poskus and colleagues examined 280 women through pregnancy and the first month after birth 1. Peri-anal problems developed in 123 women (43.9%). Of those 123, 1.6% started in the first trimester, 61% in the third trimester, 34.1% after delivery, and 3.3% at the one-month visit. Hemorrhoids accounted for almost all of it: 114 women had hemorrhoids, 7 had hemorrhoids plus a fissure, and 2 had a fissure alone. Constipation, a personal history of peri-anal disease, pushing for more than 20 minutes in labor, and a baby heavier than 3,800 grams were the predictors in their model. The confidence intervals were wide. For constipation the odds ratio was 18.98, with an interval from 7.13 to 50.54, so the exact size of that risk is unstable.

A Belgian cohort of 94 women found anal symptoms in 68%, and constipation by a standard definition in 60.7% at some point in the study 2. Constipation raised the odds of anal complaints (odds ratio 6.3, 95% confidence interval 2.08 to 19.37). A past anal problem raised them too (odds ratio 3.9). Anal pain was the most common symptom. Hemorrhoid clots were most often recorded right after birth, and prolapse in the third trimester and right after birth. The authors’ plain advice was to prevent constipation. Fiber belongs in the plan beside these products. The bleeding evidence, from seven trials, is a drop of about half in ongoing bleeding with fiber 15. Doses and product choices are in our fiber supplement roundup.

A pregnant woman in a long pink dress resting on a sofa, head on a pillow, one hand on her belly
Hemorrhoids in pregnancy cluster in the third trimester, when constipation is also common.

A Cochrane review of hemorrhoid treatments in pregnancy and the puerperium, the weeks after birth, found few usable trials 4. From ten possible studies, two made the cut, 150 women total, and both tested oral rutosides against placebo. The review authors said those drugs looked helpful for symptoms and still should not be recommended until safety is clearer. The approaches people actually use, diet changes and local treatments, had not been properly tested. A Cochrane review of constipation treatments in pregnancy was similarly small 5. Fiber, in one trial of 40 women, added about 2.24 bowel movements per week compared with no fiber. Stimulant laxatives beat bulk-forming laxatives for “feeling better” in one trial of 140 women, and they also brought more abdominal discomfort and more diarrhea. The ACOG digestive FAQ says pregnancy hormones slow the gut, tells women to aim for 25 grams of fiber a day, and describes bulk-forming laxatives as the ones thought to be safest 8.

Cold, warm water, and what was actually tested

The NHS page on piles in pregnancy tells you to eat high-fiber foods, drink water, avoid long standing or sitting, exercise, hold a cloth wrung out in iced water gently on the area, avoid straining, and clean with moist paper, then pat the skin dry 7. It also says medicines soothe irritation and do not fix the cause, and that you should ask a doctor, midwife, or pharmacist before using a cream. ACOG lists ice packs for swelling, a warm bath, and a cream or suppository, plus fiber and fluids to help prevent hemorrhoids 8.

Warm water and cold do different things in the studies, which is why this list has both. In a lab study, resting anal pressure fell after the anus was immersed at 40 degrees Celsius and did not fall at 5 degrees Celsius 12. A separate study of warm baths found pain relief that lasted longer at higher water temperatures, after about 10 minutes in the bath, and the authors tied that to the internal sphincter relaxing 16. Water that hot can burn. A 2005 review of sitz baths found little trial evidence that they help anorectal problems, and it listed burns among the reported complications 11. Use water that feels comfortably warm on your wrist. If it reddens the skin, it is too hot. Step-by-step timing is in how to take a sitz bath.

Cold has support from a different setting. After hemorrhoid surgery, a 2025 trial of 166 patients found ice packing beat warm sitz baths for pain in the first 16 hours, with less swelling at 24 hours and lower morphine use that day 10. The patients had surgical wounds, so the result only lines up with the NHS iced cloth and the ACOG ice pack. Our ice or heat explainer and the ice pack roundup go further on timing. For a new parent, cold is the first-day tool for swelling, and a warm soak is the tool for aching and spasm once you can sit safely.

Shirah and colleagues compared two routines in 495 pregnant women with hemorrhoids 6. One group, 284 women, used a warm salty sitz bath three times a day. The other, 211 women, used a topical cream twice a day. Both groups also used a glycerin suppository before bowel movements and a daily psyllium supplement mixed in 8 ounces of liquid. Everyone in the sitz group was recorded as healed, compared with 84.8% in the cream group. The study was not blinded, and both groups got fiber, so the gap may be smaller than those percentages suggest. It is still why a sitz bath belongs on this list during pregnancy as well as after birth. The pregnancy guide covers the rest of that season.

Cooling pads and the non-medicated balm do not have a trial like that. Buyers use the pads as a fold-and-leave compress for 10 to 15 minutes, including after childbirth. Whether witch hazel itself changes a hemorrhoid is a separate question, covered in does witch hazel work. The balm is for people who want to skip a medicated cream.

Creams, and the one safety study we can cite

A Motherisk study followed women who used a hydrocortisone-pramoxine foam in the third trimester, 204 exposed pregnancies and a similar number of controls 9. Exposure was not linked with differences in birth weight, gestational age, prematurity, or complications before or after birth. The authors called that foam safe in late pregnancy. Staroselsky and colleagues, writing for family doctors, said common topical hemorrhoid products have not each been assessed in pregnancy, and that harm to a third-trimester baby from the usual anesthetic, steroid, and anti-inflammatory creams is unlikely 3. They still put fiber, fluids, stool softeners, and toilet habits first. NIDDK says doctors usually limit over-the-counter hemorrhoid products to about a week, and to check back if they fail or cause a rash or dry skin 13.

None of that is a breastfeeding study. If you are nursing, ask before you start a steroid cream, a numbing cream, or a medicated pad. The non-drug options on this page do not have that question hanging over them.

Soft stools, in pregnancy or afterFiber and water, from the fiber roundup

During pregnancy, before any of the gadgets, do the NHS list: fiber foods, water, less standing still, walks, no straining, and a cold cloth if a lump is throbbing 7. A sitz routine like the one in the pregnancy study is reasonable if your clinician is fine with warm soaks 6. Save the peri bottle and the big pads for after a vaginal birth, when wiping and swelling are both at their worst. After a cesarean, you may still have pregnancy hemorrhoids, and the same rinse, cold, and fiber apply. You do not need a peri bottle for a scar that is on the abdomen.

Skip the donut cushion if you will be in a chair most of the day for weeks. The product notes say a cushion that supports the thighs is a better match for that, and our hemorrhoid pillow page compares those shapes. Skip the Fivona if your hips hang over the seat. The sitz bath roundup has a wider basin. Skip a medicated cream if you have not asked, especially in the first trimester, where the foam study does not apply 9.

A sudden very painful lump at the anal opening can be a clot in an external hemorrhoid. Cold and a cushion may take the edge off. The thrombosed hemorrhoid guide explains why the first few days are the window when a clinician might drain one.

How to use them safely

Set up a small bathroom kit before the birth if you can: the peri bottle, a stack of cold options, and psyllium you already know you tolerate. After you are home, a simple day looks like this.

  1. Rinse with warm water from the peri bottle after you pass stool or urine. Ten ounces goes quickly, so fill it again before you wipe dry skin to finish the job. Pat with a soft cloth or moist paper. Patting dry is the NHS advice 7. If the bottle leaks in a bag, empty it before you pack it, and check the cap.
  2. For swelling, use cold for the stretch the product actually stays cold. The instant pads run about 20 minutes. The Lansinoh pads run about 15 to 30 minutes. Keep a sleeve or a thin cloth between the gel and your skin. Stop if the skin numbs or hurts. You can repeat later. You do not need to ice for hours.
  3. When you want a soak, set the Fivona on the toilet, fill it with comfortably warm water, and sit for about 10 minutes, the time used in the warm-bath study 16. NIDDK says a sitz bath several times a day for pain 13. The pregnancy study used three soaks a day 6. Two or three is a reasonable home target if you are steady on your feet. Have someone nearby the first time if you are lightheaded. Drain the basin into the bowl when you stand. If the seat feels narrow, stop using it. A wobbly basin is a fall risk.
  4. Cooling pads can sit folded against the skin for 10 to 15 minutes, which is how buyers use them, or you can wipe and throw the pad away. If they stick together, peel them apart before you are in a hurry. If the skin gets more irritated, stop. Medicated pads still count as something to mention to your provider while pregnant or nursing.
  5. Use a pea-sized amount of the balm on sore external skin. It is greasy. Put it on after a rinse, and use a liner if it stains underwear. It is not a numbing cream. If you still cannot sit or sleep because of pain, you need a clinician, not a thicker layer.
  6. Sit on the ring cushion for a feed or a short meal. Stand up and walk when you can. ACOG lists long sitting among the factors that lead to hemorrhoids 8. The cushion is a break from a hard chair, not a place to spend the afternoon.
  7. Keep taking fiber with a full glass of water, and drink through the day. The pregnancy fiber result is above 5. Stay with it for days to weeks. The fiber roundup explains psyllium and the options that are easier to drink. Tell your obstetric provider you are taking it daily.
A new parent in a robe sitting on a ring cushion on a sofa, with an angled rinse bottle and a folded towel on the side table
A ring cushion is for a short feed, not a whole day in the chair.

If a soak stings, the water is too hot, the skin is already raw, or something in the water is irritating you. Empty it, rinse with plain lukewarm water, and skip soaks until the skin is calm. If cold stings the same way, stop the ice and call.

When to see a doctor instead

Most of these flares are painful and still settle with time, softer stools, and less irritation 34. A few need a procedure, or they are something else. When to see a doctor spells out the difference, including bleeding that is more than a bright red streak.

The USPSTF recommends colorectal cancer screening starting at age 45 for adults at average risk 14. Postpartum bleeding does not cancel that if you are in the screening age and overdue. NIDDK says to get care right away for severe anal pain and rectal bleeding, especially with belly pain, diarrhea, or fever 13. Fever after birth, or bleeding that soaks through pads faster than your obstetric team told you to expect, belongs with them the same day.

Frequently asked questions

How long do hemorrhoids last after birth?

Many ease in the weeks after delivery, and reviews of pregnancy hemorrhoids say most symptoms settle soon after birth. In one cohort, new peri-anal problems were uncommon by the one-month check. A painful clot or a hemorrhoid that keeps bleeding past those early weeks should be seen, not waited out.

What helps hemorrhoids while breastfeeding?

Rinsing, cold, a warm soak, fiber, water, and a non-medicated balm do not depend on a drug passing into milk. We do not have a breastfeeding trial for the medicated creams people often buy. Ask your obstetric provider or the baby's clinician before you use a steroid cream, a numbing cream, or a medicated pad.

Are sitz baths safe during pregnancy?

A large pregnancy study used warm salty soaks three times a day, along with psyllium, and recorded high healing rates. The water should be comfortably warm, not hot enough to redden the skin. Skip a soak if your clinician has told you not to sit in a tub, and use plain warm water unless they suggest something else.

Can I use ice packs for postpartum hemorrhoids?

Yes, with a cloth or the sleeve that comes with the pack. The NHS describes a cloth wrung out in iced water, held gently. ACOG lists ice packs to reduce swelling. Take the cold off if the skin goes numb or the cold starts to hurt. These packs fade in about 15 to 30 minutes.

Do donut pillows help after childbirth?

They take pressure off a sore spot for a short sit, which is why people use them during feeds. They do not treat the hemorrhoid. If you will be sitting most of the day for weeks, a cushion that supports your thighs is a better match than a ring.

When should I call about hemorrhoids after birth?

Call promptly for heavy bleeding, fever, severe pain, or a sudden hard lump. Bright red streaks on the paper can be a hemorrhoid, and black stools or bleeding with belly pain need urgent care. Your obstetric provider should also know about bleeding that worries you, even when you think it is from the hemorrhoid.
