Quick answer

A hemorrhoid pillow makes a chair hurt less. Use a donut ring for a short time on a sore spot, and a seat cushion with a tailbone cutout for desk work, driving, or a hard chair. These cushions have not been tested as a hemorrhoid treatment. Keep toilet sits short, and see a doctor for heavy bleeding or severe pain.

Key takeaways

  • No clinical trial has tested donut pillows or seat cushions as a hemorrhoid treatment, so these picks are for sitting comfort.
  • In a 2025 colonoscopy study of 125 adults, smartphone use on the toilet was linked with a 46% higher chance of hemorrhoids, and phone users were much more likely to sit longer than five minutes.
  • Among 2,813 people in a colonoscopy study, constipation was linked with more hemorrhoids, while sedentary time was linked with slightly lower odds, so chair sitting is not the same problem as long toilet sits.
  • The American College of Obstetricians and Gynecologists still lists long periods of sitting among hemorrhoid causes, and the AGA says to avoid prolonged time on the toilet.
  • A cushion that makes your legs numb, or that you use to linger on the toilet, is the wrong tool.
On this page

A hemorrhoid pillow is a sitting aid. It takes pressure off a sore spot so a chair is bearable during a flare. It has not been tested as a treatment that shrinks a hemorrhoid, and we did not find a trial that says it does. If the pain is an external hemorrhoid, the swollen vein under the skin around the anus, a cushion can make the workday possible while the swelling settles.

Which shape you want depends on how long you sit. A donut ring is for a shorter stretch with one sore spot. A seat cushion with a cutout under the tailbone is for a desk, a car seat, or a hard chair, because it still supports your thighs. If you are choosing one today, get the ring for a brief sore spell and the cutout cushion if you sit for most of the day.

The sitting that studies keep tying to hemorrhoids is sitting on the toilet, not sitting in an office chair. Keep those visits short. A cushion on the toilet seat, used so you can stay longer, works against you. Toilet habits that help hemorrhoids cover the short sit, the footstool, and gentler cleaning.

Our picks compared

Five cushions cover a donut for a sore spot, a workday seat, a larger ring, a cushion you can carry into the car, and a firm pad for hard chairs.

Reviews of our picks

The notes below come from how each cushion is built and from complaints that repeat. Firmness is the split on almost every one of them.

Best donut pillow
Kieba (Cushion)

Kieba Donut Cushion, 18 inch

Best for: Short-term relief from a sore, swollen spot

Ring shapeWashable coverNon-slip base

This dense foam ring is for a sore, swollen spot when you still have to sit for a while. Buyers who drive, study, or work at a desk often say it stays comfortable and holds its shape, and the cover comes off to wash. It is too firm for some people. If you will be sitting most of the day for weeks, a cushion that supports your thighs is a better fit.

What recent buyers say

Truck drivers, students and desk workers all say it made a real difference. A September reviewer had tried a memory foam ring from a pharmacy and found this one far more comfortable. Firmness divides people: most find the balance right, but a sizeable minority say it is too hard. One recent buyer thought it was small.

What buyers like

  • Comfortable for hours of sitting
  • Holds its shape
  • Cover comes off for washing

Common complaints

  • Too firm for some people
  • Smaller than some expect

Skip it if you will be sitting most of the day for weeks. A contoured seat cushion that supports your thighs may suit you better.

Best for desk work
Everlasting Comfort (Cushion)

Everlasting Comfort Memory Foam Seat Cushion

Best for: Office chairs and long workdays

U-shaped cutoutMemory foamWashable cover

The open back takes pressure off the tailbone while the rest of the cushion holds your thighs, which is what a full workday needs. Buyers describe comfort and help with back and tailbone pain, and the non-slip cover stays put. Firmness splits people, and many who mention it wish the foam were softer.

What recent buyers say

The review count is huge and the story is consistent: comfortable, helps back and tailbone pain, and worth the money. Firmness is the recurring split, with about half of the people who mention it wishing it were softer.

What buyers like

  • Comfortable for a full workday
  • Good support for back and tailbone
  • Non-slip cover stays put

Common complaints

  • Too firm for some people

5 STARS UNITED Large Donut Pillow

Best for: Larger bodies, or hemorrhoids plus tailbone pain

Large sizeTailbone cutoutsMesh cover

This is a larger ring with extra cutouts at the back, for a bigger body or for hemorrhoids plus tailbone pain. Buyers say they can sit for long stretches without going numb, and that it holds its shape for most people. It can feel very hard at first, and some people find it flattens over time.

What recent buyers say

Buyers say they can sit for hours without going numb, and that it holds its shape and is worth the money. Firmness is the main split. Some find it cushy, others call it hard as a rock at first. A share of reviewers say it flattens over time.

What buyers like

  • Comfortable for long stretches
  • Good value
  • Holds its shape for most people

Common complaints

  • Feels very firm at first

A gel top and a handle make this the cushion to move between the car, the office, and home. Recent buyers describe comfort in the car, and some carry it onto planes and into restaurants. It is well made. Like the other foam cushions, some people find it too firm.

What recent buyers say

Best for hard chairs
Aylio (Cushion)

Aylio Coccyx Seat Cushion

Best for: Hard chairs, and heavier users who flatten softer foam

Firm foamU-shaped cutoutWashable cover

Firm, springy foam that long-term owners say keeps its shape for years, including heavier users who flatten softer cushions. It suits hard chairs, and it has a carry handle. It is not a plush pillow, and the cover can wear at the edges after a long time.

What recent buyers say

Owners report years of daily use, and one heavier buyer says it holds up well. People like the medium-firm feel and the carry handle. It takes some trial and error to place on different chairs, and a few found the cover worn at the edges after several years.

What buyers like

  • Keeps its shape for years
  • Medium-firm support
  • Works well on hard chairs

Common complaints

  • Not as soft as some expect
  • Cover wears at the edges with age

How we chose these hemorrhoid pillows and cushions

We looked for cushions with strong overall ratings from a large number of buyers, including two seat cushions that are among the most reviewed on Amazon. We read recent reviews for the same praises and the same complaints. Comfort for long sits shows up constantly. So does “too firm.” A few listings mention flattening over time, or a cover that wears at the edges after years of use.

We then asked what the research can support. There is no trial of a donut pillow or a coccyx cushion (a cushion with a cutout for the tailbone) as a hemorrhoid treatment. The picks are matched to a job buyers describe, not to a cure. The ring is for a sore spot you need to unload for a while. The open-back seat cushions are for people who sit for hours and still need thigh support. The larger ring is for bigger bodies, or for hemorrhoids plus tailbone pain. The gel cushion is the one with a handle, for the car and for carrying one pad between rooms. The firm foam pad is for hard chairs and for people who squash softer foam.

We did not rank by price. We did drop the idea that one shape fits every chair.

What the research says about sitting and hemorrhoid cushions

Be precise about which kind of sitting the studies mean.

The American College of Obstetricians and Gynecologists lists standing or sitting for long periods among the causes of hemorrhoids, along with pregnancy, straining, constipation, and being overweight 1. That is standard patient advice. A larger study does not show the same pattern for ordinary sedentary time. Among 2,813 people who had a colonoscopy in a prevention trial, 1,074 had hemorrhoids recorded. Constipation was linked with higher odds (odds ratio 1.43). Sedentary behavior was linked with slightly lower odds (odds ratio 0.80), and physical activity was not clearly linked either way. A 2015 study of hemorrhoid risk factors on screening colonoscopy is the source of those figures 2. High grain-fiber intake was linked with lower odds. In that study, more sedentary time was not linked with more hemorrhoids.

Toilet time is different. MedlinePlus lists sitting for long periods, especially on the toilet, among hemorrhoid causes, and it says too much straining, rubbing, or cleaning can make symptoms worse 3. The AGA clinical guidance on hemorrhoids calls fiber and avoiding straining or prolonged toilet time reasonable first-line care 5. The NHS page on piles says not to spend more time than you need on the toilet 6.

A 2025 study of smartphone use on the toilet looked at 125 adults having a screening colonoscopy 4. Hemorrhoids were seen in 43%. Two-thirds of the group used a phone on the toilet. Among phone users, 37.3% spent more than five minutes per visit, compared with 7.1% of people who did not use a phone. After adjustment for age, sex, body mass index, exercise, straining, and fiber, phone use was associated with a 46% increased risk of hemorrhoids. This was a cross-sectional study, a snapshot, so it cannot prove the phone caused the hemorrhoids. It does show that phone use and long sits travel together. Our article on scrolling on the toilet walks through that study.

What does treat bleeding, in actual trials, is fiber. A 2006 meta-analysis of seven fiber trials in 378 people found fiber cut the risk of bleeding by about half (relative risk 0.50) and lowered the chance that symptoms would persist 7. Pain and itching might improve too, but those results were uncertain. A cushion does not replace that.

Donut pillow or seat cushion?

The shape is the whole decision. We did not find a head-to-head trial.

A ring has a hole in the middle so a small sore spot can sit in the opening instead of under your weight. The Kieba is an 18 inch ring in dense foam, with a washable cover and a non-slip base. Truck drivers, students, and desk workers are the buyers who say it made sitting possible. One person who had tried a memory-foam ring from a pharmacy found this firmer one more comfortable. Firmness still divides people. A sizeable minority call it too hard, and some find it smaller than they expected. The product notes are direct: if you will sit most of the day for weeks, a cushion that supports your thighs may suit you better.

The 5 STARS UNITED ring is larger, with extra cutouts toward the tailbone. Buyers say they can sit for hours without going numb, and that it holds its shape. The first sits can feel hard as a board. A share of reviewers say it flattens later. Skip it if you want a soft, sink-in feel. Choose it if you have a larger body, or if tailbone pain and a hemorrhoid are both in the way.

A U-shaped or open-back cushion does the opposite of leaving you perched on a ring. The cutout unloads the tailbone, and the foam under your thighs carries your weight. That is the better desk tool.

A person works at a wooden desk in an office chair fitted with a teal U-shaped seat cushion
The cutout sits at the back, under the tailbone, while the rest of the cushion supports your thighs.

The Everlasting Comfort cushion is the workday pick. The review story is steady: comfortable, helpful for back and tailbone pain, and a cover that does not slide. About the firmness comments, many people wish it were softer. If your chair is already soft, this can feel like too much foam. If your chair is a flat office seat, it is the one to try first.

The ComfiLife cushion adds a gel top layer and a handle. Recent buyers describe relief in the car, and some take it through hip-surgery recovery, onto planes, and into restaurants. The handle is why it wins for driving and for one cushion that moves around. Too firm is again the caveat. Do not start a long drive if you cannot sit without severe pain even on the cushion. A pad is not a reason to push through pain that needs a doctor.

The Aylio is firmer springy foam for hard chairs, and for heavier users who flatten softer cushions. Owners report years of daily use. The feel is medium-firm, and there is a handle. It can take trial and error to place it so the cutout lines up on different chairs. After several years, some covers wear at the edges. A worn cover slides. The foam is the part people keep.

How to place it

  1. Put the cushion on the chair before you sit. The cutout, if there is one, goes to the back, under the tailbone. On a ring, the hole goes under the sore spot, not under one thigh.
  2. Your feet should still reach the floor. If a thick cushion leaves you dangling, it is the wrong height for that chair.
  3. Sit, and notice numbness. Tingling or a leg that falls asleep means the edge is pressing a nerve or the ring is too hard. Stand up. Try a shorter sit, or a different shape.
  4. Stand when the spot throbs. There is no studied “correct” number of minutes in a chair for hemorrhoids. The Peery study did not find that moving more prevented hemorrhoids 2. Standing is how you stop pressing on a sore spot, not a proven cure. Our guide to exercise with hemorrhoids covers workouts that are kinder during a flare.
  5. Wash the cover when it is soiled. Several of these covers come off. A dirty pad against irritated skin makes itching worse.

A cushion with a handle is the practical one if you move it from the car to a desk.

How to use a hemorrhoid pillow during a flare

Use the cushion for the sitting you cannot cancel. Keep the toilet on a different rule.

Go when you need to. Do not wait until you are straining, and do not stay after you are done. What causes hemorrhoids lays out straining, constipation, and long toilet sits side by side. A footstool that raises your knees can shorten the strain of a bowel movement. That is a different product. Our toilet stool picks are for the bathroom, not for the office chair.

At night, a ring does not belong under your hip in bed. Side sleeping with a pillow between the knees takes pressure off in a different way. The best sleeping position for hemorrhoids is the page for that.

If the cushion helps you sit, you still need a plan for the bowel movement that caused the flare. Fiber is the treatment with trial evidence for bleeding 7. NIDDK says to follow up if home products have not helped after a week 8. A week of worsening pain or bleeding is a reason to be examined. How to prevent hemorrhoids is the habit side: fiber, fluids, toilet time, and movement.

Pregnancy and larger bodies

Pregnancy raises the chance of hemorrhoids. In one cohort of 280 pregnant women, 40.7% were diagnosed with hemorrhoids, and constipation was a strong predictor 11. A chair cushion can make sitting easier. It will not soften a stool. Read hemorrhoids during pregnancy before you add creams, and talk with your obstetric provider about bleeding.

For a larger body, start with the bigger ring if you need a hole under a sore spot and you also have tailbone pain. Start with the Aylio if the problem is a hard chair and softer foam collapses under you. Neither listing publishes a weight limit, so fit is something you learn by sitting, not from a chart.

A clot is not a cushion problem

A sudden hard lump that is extremely painful can be a thrombosed hemorrhoid, a clot in an external hemorrhoid. A review notes that excision can greatly reduce pain if it is done in the first two or three days of symptoms 10. A pillow may get you to that appointment. It should not delay it. Read what a thrombosed hemorrhoid is if the lump appeared suddenly.

When a cushion is the wrong tool

Stop using it, and get care, if sitting is getting worse, if you go numb every time, or if there is heavy bleeding. The US Preventive Services Task Force recommends colon cancer screening starting at age 45, and more strongly from 50 to 75 9. Rectal bleeding at that age needs a look even when you also have hemorrhoids.

Our page on when to see a doctor for hemorrhoids separates symptoms that can wait from symptoms that should not. If a cushion has not made sitting tolerable after a few days, the next step is an exam, not a second pillow in a different color.

Frequently asked questions

Do donut pillows make hemorrhoids worse?

No study we found shows that a donut pillow makes hemorrhoids worse. No study shows that it shrinks them either. A ring leaves a hole under the sore spot, which is why people buy one. If the ring digs into your thighs or your legs tingle, switch to a cushion that supports your thighs and has a cutout at the back.

Can I take a hemorrhoid pillow onto the toilet?

Use it on a chair, not as a way to sit longer on the toilet. Long toilet sits are the sitting habit linked with hemorrhoids. The cushion belongs at the desk, in the car, or on a hard chair you cannot avoid. Get up when the bowel movement is done.

How long can I sit on a hemorrhoid cushion?

Sit for as long as you are comfortable, and stand when the spot starts to throb. A donut is a better fit for shorter sits. A seat cushion with a tailbone cutout is the one people use for a workday. Numbness is a reason to get up and to try a different cushion.

Are hemorrhoid pillows safe in pregnancy?

A cushion is not a medicine. It can make a chair easier in pregnancy, when hemorrhoids are common. It will not treat constipation, which matters more. Tell your obstetric provider about bleeding or a new painful lump.

What if the cushion feels like a rock?

That complaint shows up often, especially with firmer rings. Try shorter sits for a few days. If it still hurts, choose a different shape. Staying on a cushion that hurts will not train the hemorrhoid to settle.

Will a pillow help a thrombosed hemorrhoid?

It may let you sit while you arrange care. It will not remove the clot. A sudden hard lump that is very painful, especially in the first few days, deserves a same-day medical visit. Removal in that early window can shorten the pain.
