Quick answer
Psyllium powder has the strongest evidence for hemorrhoid bleeding and for constipation. For most people we would start with sugar-free orange Metamucil, mixed with a full glass of water. Give it about four weeks. Plain husk, packets, capsules, or gummies make sense only if that is the form you will actually keep taking.

Key takeaways
- In a review of seven trials, fiber cut the risk of hemorrhoid bleeding by about half compared with no fiber.
- A 2022 review of constipation trials found a benefit mainly with psyllium, doses above 10 grams a day, and at least four weeks of use.
- The Dietary Guidelines use 14 grams of fiber per 1,000 calories, about 28 grams on a 2,000-calorie day, and most Americans fall short.
- Mix psyllium powder with 8 ounces of liquid and drink at least 8 ounces with every dose. Gas is the usual side effect.
- MedlinePlus says not to take psyllium longer than a week unless a doctor agrees. The hemorrhoid trials ran for weeks to months, so tell your clinician if daily fiber is the plan.
On this page
Fiber is the home treatment with the best evidence for hemorrhoid bleeding. In a 2006 meta-analysis of seven fiber trials, people who took fiber were about half as likely to keep bleeding as people who did not 1. If you buy one product, buy a psyllium powder you will actually drink. For most adults that is sugar-free orange Metamucil.
Fiber softens the stool so you strain less. That is the job the NIDDK treatment page gives it, alongside fluids and shorter toilet sits 8. A large prolapsed hemorrhoid can still need more than a powder. The food version of the same idea is in our hemorrhoid diet guide. A supplement covers the gap when meals do not.
This page matches six well-reviewed supplements to that evidence: who each one suits, how much the trials used, how to take psyllium without choking on a gel, and what to do if a month of fiber does not change the bleeding.
Our picks compared
Psyllium powders sit at the top because that is the fiber the constipation trials support. The other three are here for people who will not drink psyllium.

Metamucil Sugar-Free Psyllium Powder, Orange
Best for: Most people starting fiber for hemorrhoids
This is the psyllium drink most people can stay with. The orange flavor goes down more easily than plain husk, a tub lasts, and psyllium is the fiber named for hemorrhoids on the NIDDK treatment page and used in the stronger constipation trials. It suits you if you will mix a glass every day. Skip it if you want no sweetener and no coloring.
What recent buyers say
Reviews are strongly positive. It works, it tastes better than people expect, and a tub lasts. Recent buyers say it made them regular within a day or two, and they stress drinking plenty of water with it. The main complaint is the added sweetener and coloring, which some would rather avoid.
What buyers like
- Keeps people regular
- Orange flavor goes down easily
- Big tub is good value
Common complaints
- Some dislike the sweetener and coloring
- Thickens if you do not drink it quickly
Skip it if you prefer plain fiber with nothing added. Anthony's plain psyllium is the unflavored option.
Anthony's Organic Psyllium Husks, 1.5 lb
Best for: People who want plain fiber with nothing added
Anthony's is plain psyllium husk with nothing added, so it fits people who do not want flavoring. Buyers say it keeps them regular about as well as the big brand, and you can stir it into oatmeal or a smoothie. The husks stay gritty and do not fully dissolve, so skip it if texture is what made you quit fiber before.
What recent buyers say
Buyers say it works about as well as the big brand, and many like that it is plain. Texture is the split. Some find it smooth, others gritty, and whole husks never dissolve the way a fine powder does. A few found grit at the bottom of the bag.
What buyers like
- Works as well as name brands
- Nothing added
- Low cost per serving
Common complaints
- Gritty for some
- Does not dissolve fully
Konsyl Daily Psyllium Sticks, 30 pack
Best for: Travel, and anyone who wants an exact dose every time
Each stick is 5 grams of plain psyllium, so the dose stays the same at home or in a bag. Recent buyers use the packets for travel, including people who got tired of loose powder in a carry-on. The powder gels quickly and can feel gritty, so drink it as soon as you stir it. One stick is below the daily dose that worked best in the constipation review, so some people use two.
What recent buyers say
Recent buyers love the packets for travel and for keeping a big tub off the counter. One traveler switched after airport security kept questioning a bag of loose powder. Texture is the drawback. It gels quickly and can feel gritty, and one reviewer could not get it down at all.
What buyers like
- Portable single servings
- No mess
- Plain, with no sugar
Common complaints
- Gritty, clumpy texture
- Gels quickly in water
Benefiber Unflavored Fiber Powder, 250 servings
Best for: People who cannot stand the texture of psyllium
Benefiber disappears in water or coffee and has almost no taste, which is why people who hate psyllium stick with it. Buyers say it keeps them regular and is gentle on the stomach. It is wheat dextrin, a different fiber from the psyllium in the hemorrhoid and constipation trials, so we rank it behind psyllium when bleeding is the problem.
What recent buyers say
Buyers say it vanishes into water or coffee with no taste or grit, and many find it keeps them regular without upsetting their stomach. The big tub is good value. There is no recurring complaint in the reviews.
What buyers like
- No taste or grit
- Dissolves in coffee and water
- Gentle on the stomach
Metamucil Fiber Gummies, Orange
Best for: People who will only take fiber if it tastes good
These orange gummies are for anyone who will not drink a fiber mix. A serving supplies 5 grams, which is under the dose that helped most in the constipation review, and some buyers get gas or bloating. The gummies also stick together in the jar. They earn a place when taste is the only way fiber will happen at all.
What recent buyers say
September 2026 reviews praise the orange taste and say they are easy to take. Effects on digestion are mixed. Some stay regular, others get gas or bloating. The gummies stick together in the jar, and they cost more per gram of fiber than powder.
What buyers like
- Tastes good
- Easy to take
- Convenient
Common complaints
- Gas or bloating for some
- Stick together in the jar
- Costly per gram of fiber
Kirkland Signature Fiber Capsules, 360 count
Best for: Psyllium capsules on a budget
These are psyllium capsules, the same fiber family as the powders, and buyers say they work about as well as the name brand with no taste. They suit you if swallowing pills is easier than mixing a drink. The complaint that comes up over and over is how many capsules a full dose takes.
What recent buyers say
Reviewers say they work as well as the name brand and go down easily with no taste. The complaint is how many capsules you need for a full dose.
What buyers like
- As good as the name brand
- Easy to swallow
- Great value
Common complaints
- Many capsules per dose
How we chose these fiber supplements
We started with fiber supplements that already had strong overall ratings from a large number of buyers, then read recent reviews for the same praises and the same complaints. Taste, texture, travel, and how many capsules a dose takes showed up again and again. Those patterns decided the awards. We did not use a single glowing review, and we did not let a brand pay for a slot.
The research decided the order. Psyllium has trial evidence for softer stools and, in the hemorrhoid review, for less bleeding 12. Wheat dextrin and a small gummy dose do not have that same record, so they rank lower even when buyers like them. Konsyl has fewer reviews than the household names. We kept the sticks because recent buyers keep describing the same practical use: a set dose that survives a trip.
What the research says
The hemorrhoid evidence comes from one systematic review of seven trials. Alonso-Coello and colleagues pooled seven randomized trials with 378 patients 1. Fiber lowered the risk of symptoms that failed to improve (relative risk 0.53, 95% confidence interval 0.38 to 0.73) and lowered the risk of bleeding by half (relative risk 0.50, 95% confidence interval 0.28 to 0.89). The bleeding result is real, and the confidence interval is wide, so the true effect could be smaller than half. Pain, itching, and prolapse leaned in fiber’s favor, but those intervals were wide enough that no effect is still possible. Studies that checked again, usually at six weeks and at three months, saw the same pattern. Mild side effects were a bit more common with fiber, and that difference was not statistically significant.

A later look at constipation, which is the problem fiber is trying to fix, is larger. A 2022 meta-analysis of 16 randomized trials included 1,251 adults 2. About 66% of people on fiber improved, compared with 41% on the control treatment (relative risk 1.48). Psyllium and pectin were the fibers with a clear effect. The benefit showed up at doses above 10 grams a day. Stool frequency improved when people stayed on fiber for at least four weeks. Flatulence was higher in the fiber groups. The authors warned that the trials disagreed with each other a lot, so treat the average as a rough guide.
One head-to-head trial compared psyllium with the stool softener docusate sodium in 170 adults with chronic constipation 3. Psyllium was 5.1 grams twice a day. Docusate was 100 milligrams twice a day. After two weeks, psyllium increased stool water more than docusate, and bowel movements averaged 3.5 per week on psyllium versus 2.9 on docusate. That is the main reason our stool softener, fiber, or laxative comparison puts fiber first for hemorrhoids, with softeners as a short bridge. The stool softener roundup is there if a clinician wants a softener for a few days.
A smaller US trial gave adults with chronic constipation psyllium at 12 grams a day, prunes, or kiwifruit for four weeks 4. All three raised the number of complete bowel movements. Straining improved on psyllium too. Side effects were most common in the psyllium group. Kiwifruit caused the least upset, which matters if gas is why you quit fiber last time. Prunes and kiwifruit are food, and they belong in the hemorrhoid diet alongside a supplement.
Water is part of the dose. In a two-month trial, 117 adults ate about 25 grams of fiber a day 5. The group told to drink about 2 liters of mineral water had more bowel movements and used fewer laxatives than the group drinking about 1.1 liters. The NIDDK hemorrhoid diet page says liquids help the fiber in your diet work, and that you should ask a clinician how much to drink for your health and where you live 7.
Why the target is higher than a supplement serving
The Dietary Guidelines for Americans, cited on that NIDDK page, set fiber at 14 grams per 1,000 calories 7. On a 2,000-calorie day that is 28 grams. The American College of Obstetricians and Gynecologists tells women to aim for 25 grams a day and notes that many Americans get about 15 9. A separate review of national surveys put the share of people who meet fiber recommendations at about 5% 10. A supplement of 5 to 12 grams closes part of that gap. Meals still have to carry most of the day’s fiber. Our fiber calculator adds up a normal day so you can see the hole before you pick a powder.
Constipation and hemorrhoids travel together more often than an old survey suggested. In a colonoscopy study of 2,813 adults, constipation was more common in people who had hemorrhoids (odds ratio 1.43), and a higher intake of grain fiber went with a lower risk (odds ratio 0.78) 6. That study enrolled people already coming in for screening, so it cannot tell you that a jar of powder prevents every flare.
Psyllium, wheat dextrin, and gummies
NIDDK names two supplement fibers for hemorrhoids: psyllium and methylcellulose 8. Our list stays with psyllium products plus two alternatives buyers actually finish. Benefiber is wheat dextrin. A 2016 review concluded that readily fermented soluble fibers such as wheat dextrin do not increase stool water or stool output, while gel-forming psyllium does 13. One author of that review works at the company that makes Metamucil, so read it with that in mind. Buyers of Benefiber still say it keeps them regular. We still rank psyllium first for bleeding, because that is the fiber the hemorrhoid trials were testing 1.
The gummies are a blend, and the listing states 5 grams of fiber per serving. That is half of the 10-gram line from the constipation review 2. People who get gas from them are describing a side effect the trials already measured. Use them if taste is the only way you will take fiber. Do not expect one small serving to match a psyllium powder.
How to choose
Match the product to what you will take on a boring Wednesday.
| If this is you | Start here |
|---|---|
| You want the fiber with the trial record and a flavor you can drink | Sugar-free orange Metamucil |
| You want psyllium with nothing added | Anthony’s husks, in oatmeal or a smoothie if the grit bothers you |
| You travel, or you want the same dose every time | Konsyl sticks, often two a day if you are aiming above 10 grams |
| Psyllium’s texture makes you gag | Benefiber in coffee or water |
| You will only take fiber if it tastes good | Metamucil gummies, knowing the dose is small |
| Swallowing pills is easier for you than mixing a drink | Kirkland capsules, accepting that a full dose is several capsules |
Skip a powder if swallowing is already hard. Psyllium swells when it meets liquid. MedlinePlus lists trouble swallowing among the effects that need a clinician 14. People who prefer no sweetener should skip the orange Metamucil and use Anthony’s or Konsyl. Severe outside pain is a cream problem, or a problem for a clinician. Our treatment overview lays out creams, office procedures, and surgery next to home care.
During pregnancy, fiber is a reasonable first step for constipation, which raises the chance of hemorrhoids. A Cochrane review found one small trial, 40 women, in which fiber supplementation added about 2.24 bowel movements per week compared with no fiber 11. Stimulant laxatives helped constipation more than bulk-forming laxatives in another pregnancy trial, and they also caused more abdominal discomfort and diarrhea. ACOG describes bulk-forming laxatives as the ones thought to be safest 9. Run any new daily supplement past your obstetric provider. Our pregnancy hemorrhoid guide covers the rest of what is considered safe.
How to use them safely
MedlinePlus instructions for psyllium say to mix powder or granules with 8 ounces (240 milliliters) of liquid right before you drink it, and to drink at least 8 ounces of liquid with the dose 14. That includes capsules: the page says you need that liquid for psyllium to work and to limit side effects. Do not swallow dry powder. If the drink sits, psyllium turns thick. Buyers of the orange powder and of the Konsyl sticks both describe that. Stir and finish it.
Psyllium is usually taken one to three times a day. Follow the package. A practical start, lined up with the trial doses, looks like this:
- Days 1 to 3: one small serving once a day, with 8 ounces of liquid, plus your usual drinks.
- After that: move toward a daily total above 10 grams of psyllium if your stomach allows it. For Konsyl, that is more than one 5-gram stick. For a flavored powder, use the scoop on the label and add up the grams.
- Keep going for four weeks before you judge it, unless bleeding, pain, or side effects tell you to stop sooner 24.
- Take other medicines at a different time. MedlinePlus says not to take digoxin, aspirin, or nitrofurantoin within 3 hours of psyllium, and to tell your pharmacist about the rest of your medicines because doses sometimes need a change 14.

Gas is the expected downside 2. If you balloon, cut the dose in half for several days, then climb again. A hot drink or a short walk after a meal is fine. Do not “push through” severe cramps.
If four weeks of psyllium and water have not softened your stools, look at toilet habits next. Straining and long sits keep hemorrhoids angry even when the diet is decent. Our toilet habits guide covers a footstool, a time limit, and how to clean without scraping sore skin. Prevention after the flare settles is the same trio, written out in how to prevent hemorrhoids.

Fiber, stool softeners, and laxatives for hemorrhoids: what the McRorie trial, fiber reviews, and the AGA/ACG constipation guideline found.
When to see a doctor instead
Bright red blood on the paper can be a hemorrhoid, and it can be something else. Fiber is a treatment for the first situation once a clinician is satisfied about the second. Read what bleeding from hemorrhoids looks like and when to see a doctor if you are unsure which camp you are in.
The USPSTF recommends colorectal cancer screening starting at age 45 for adults at average risk 12. New rectal bleeding at that age, or older, needs a proper look even if you are sure it is a hemorrhoid. NIDDK says to get care right away for severe anal pain and rectal bleeding, especially with belly pain, diarrhea, or fever 8.
Frequently asked questions
How fast does a fiber supplement work for hemorrhoids?
Some people notice a softer stool within a day or two. The constipation trials that measured stool frequency looked at two to four weeks, and the hemorrhoid trials checked bleeding around six weeks and again at three months. If nothing has changed after four weeks of a real psyllium dose plus water, it is time to change the plan.
Is psyllium better than a stool softener for hemorrhoids?
For softening stool, one trial found psyllium beat docusate sodium over two weeks. It increased stool water and, by the second week, bowel-movement frequency. Stool softeners still have a short-term role when a doctor wants a gentle push for a few days. Fiber is the option with evidence for hemorrhoid bleeding.
Can I take fiber supplements while pregnant?
Fiber is one of the constipation treatments studied in pregnancy. In one small trial, a fiber supplement added a little over two extra bowel movements a week compared with no fiber. Check the plan with your obstetric provider, drink water with every dose, and do not start a stimulant laxative on your own.
Do fiber gummies have enough fiber for hemorrhoids?
A serving of the Metamucil gummies on this list has 5 grams of a fiber blend, not psyllium. A 2022 review saw a clearer constipation benefit above 10 grams a day. Gummies can close part of the gap if you will not take a powder. They are a weak stand-in for a psyllium dose.
What if fiber makes me bloated?
Gas and bloating are the side effects the trials actually found. Drop to a smaller amount for a few days, keep drinking water, and build back up. Stop and call a clinician if you get trouble swallowing, a rash, vomiting, or severe stomach pain.
Should I stop fiber once the bleeding stops?
Bleeding often returns when stools get hard again. The hemorrhoid trials that checked more than once still saw a benefit at three months. MedlinePlus says not to take psyllium past a week unless a doctor tells you to, so if you want it as a daily habit, say so at your next visit.
