Quick answer

Walking is a reasonable workout with hemorrhoids. Pause heavy lifting and breath-holding during a flare, and be wary of a sudden hard training week. A large colonoscopy study did not find that exercise prevented hemorrhoids. After hemorrhoid surgery, full activity may take 2 to 4 weeks.

Key takeaways

  • The NIDDK lists frequent heavy lifting as a cause of hemorrhoids and tells you to avoid regular heavy lifting, while regular activity is one of its home steps for constipation.
  • In a colonoscopy study of 2,813 adults, physical activity was not clearly linked to hemorrhoids, and sedentary time was linked to slightly lower odds.
  • A survey of 312 people who exercised at least twice a week found bodybuilding stayed linked to self-reported hemorrhoids after other factors were accounted for, while cycling's higher raw rate did not stay linked.
  • In the HubBLe trial, mean pain after rubber band ligation was 3.4 the next day and 1.6 at one week.
  • The American Society of Colon and Rectal Surgeons patient page says full activities after hemorrhoid surgery may take 2 to 4 weeks, and most surgeons want a bowel movement within 48 hours.

You can exercise with hemorrhoids. The workout that helps is usually a walk. The one that stirs a flare is a hard push: a heavy lift, a breath-hold, or a sudden jump in training. The NIDDK constipation treatment page says regular physical activity may ease constipation 1. The same institute lists frequent heavy lifting as a cause of hemorrhoids and tells you to avoid regular heavy lifting 23.

This page separates those two ideas. It covers why activity is in the plan, what is reasonable during a flare, lifting and breath-holding, cycling, yoga and the pelvic floor, and the return after banding or surgery. Toilet straining is the indoor version of the same push, on the toilet habits page. The rest of the daily plan is the prevention overview.

None of the studies below is a trial that assigned hemorrhoid patients to a workout program and counted flares. The advice is built from cause lists, constipation guidance, a few observational studies, and the recovery notes surgeons publish.

Why activity is part of the plan

Constipation shows up with hemorrhoids often enough that treating it is part of prevention. Activity is one home step the NIDDK names for constipation, next to fiber, fluid, and a regular bathroom time 1. The American Society of Colon and Rectal Surgeons patient page puts exercise in the same base of care as fiber, a supplement, and fluids. The goal is a soft, bulky stool, and fewer swings between hard stool and diarrhea 4. Meals and gram targets are on the hemorrhoid diet.

Regular walking is the activity the NIDDK names for constipation, which is the bowel problem most tied to straining.

Whether exercise prevents hemorrhoids themselves is a weaker claim, and the studies disagree.

In a colonoscopy study of 2,813 adults, 1,074 had hemorrhoids recorded. Physical activity was not clearly linked (odds ratio 0.83, and the range crossed “no difference”). Sedentary behavior was linked to about 20 percent lower odds (odds ratio 0.80) 5. A desk job, by itself, was not the pattern in that study. A 2015 colonoscopy study of hemorrhoid risk factors is the paper.

A 2026 genetic study points a little the other way. Leisure screen time was tied to slightly higher odds of hemorrhoidal disease (odds ratio about 1.05). After the authors removed outlier genetic markers, moderate-to-vigorous activity in leisure time was tied to slightly lower odds (odds ratio 0.90, interval 0.814 to 0.995). Alcohol intake was not clearly linked 8. These are small shifts from inherited tendencies, not from a 12-week training plan. A 2026 genetic study of activity and hemorrhoids is that analysis.

What is fine during a flare

Keep moving if the movement does not make the pain spike. A walk is the default. Stop when sitting, bouncing, or bracing makes the hemorrhoid throb, and try again the next day.

A French survey of 1,033 patients and 1,028 comparison patients found a more specific risk than “any exercise.” Harder-than-usual physical activity in the two weeks before the visit was more common in people having a hemorrhoid flare (odds ratio 2.79), along with constipation, a spicy diet, and alcohol. The authors counted a link at a looser cutoff than most trials (P under 0.1) 6. A steady walk is not what that survey measured. A new hard week is closer.

During a flareWhy
Walking on flat groundIt is the activity named for constipation, and it does not ask you to brace 1
Light strength sets you can finish while exhalingHeavy lifting is the pattern the NIDDK says to limit 23
A short bike ride only if the saddle does not hurtCycling’s link to hemorrhoids did not hold up after adjustment in one athlete survey 7
Warm soaks if pain is the reason you are skipping the walkSoaks are on the NIDDK home list for pain 2

The soak instructions, including how long to sit in the water, are in how to take a sitz bath. For a desk chair or a car seat while you are sore, a cushion is about comfort. It does not shrink a hemorrhoid. Shapes for desks, driving, and hard chairs are on the hemorrhoid pillow page.

Heavy lifting and holding your breath

Frequent heavy lifting is on the NIDDK cause list. Avoiding regular heavy lifting is on the prevention list 23. The page does not give a weight limit in pounds. It does put lifting next to straining on the toilet.

If you hold your breath and bear down to finish a lift, you are doing that hard push in the gym. During a flare, lighten the load until you can breathe out through the effort. Save a max attempt for after pain and bleeding have settled. The toilet habits guide is the same push in a different room.

Bodybuilding showed up in a 2025 questionnaire of people who already exercised. Researchers surveyed 312 adults, competitive and not, who did at least one sport at least twice a week. Thirty-four percent said they had hemorrhoidal disease. Among people who did bodybuilding, 48 percent said they had it. In the adjusted analysis, age and bodybuilding were the associations that stayed statistically significant 7. This was a self-report survey, not an exam, and it was a pilot. It fits the NIDDK warning about heavy lifting better than it fits a ban on all strength work.

Cycling and the seat

The same athlete survey grouped cycling with horseback riding. In that combined group, 57 percent said they had hemorrhoids. The paper’s conclusion mentions a positive association with cycling, horseback riding, and bodybuilding. The adjusted statistics told a narrower story: age and bodybuilding remained significant, and the cycling group did not get its own clear result after adjustment 7. A 2025 questionnaire on sport and hemorrhoids is the study. Read both sentences before you sell the bike.

An adult in a teal jacket rides an upright bicycle on a flat path, fully clothed, hands on the handlebars, sitting upright
A sore flare is a reason to skip a painful saddle for a few days. The athlete survey did not prove that cycling itself causes hemorrhoids.

If the saddle hurts, you do not need a study to get off the bike for a few days. Standing on the pedals for part of a ride, or waiting until you can sit without pain, is a comfort choice. A wider saddle is the same kind of choice. Neither has a hemorrhoid trial behind it. When the flare is over, the survey is not strong enough to say you must quit cycling.

Horseback riding was in that same combined 57 percent, with the same limit: it was not the result that survived adjustment 7. If you ride, use the pain as the guide during a flare, the way you would with a bike.

Yoga and the pelvic floor

No hemorrhoid trial of yoga is cited on this page, and the NIDDK does not list a pose sequence. What it does say is that regular activity may help constipation, and that people whose muscles will not coordinate a bowel movement may be offered biofeedback to retrain those muscles 1. Biofeedback is a clinic session with a sensor, not a stretch you copy from a video.

During a flare, skip positions that ask you to brace your belly and hold your breath. A forward fold that makes you strain is the gym problem again. A walk remains the movement with a sentence of support in the constipation guidance 1. If you suspect the pelvic floor is the reason stool will not pass, that is a conversation with a doctor, not a reason to add more squeezing.

Getting back to exercise after banding or surgery

Office banding and excisional surgery are different recoveries. The procedure comparison is in hemorrhoid procedures. What to expect day by day is in rubber band ligation and the hemorrhoidectomy recovery timeline.

After banding

The NIDDK says a doctor places a band at the base of an internal hemorrhoid, the band cuts off the blood supply, and the banded part shrivels and falls off, most often within a week 2. In the HubBLe trial, 187 people were assigned to rubber band ligation. Mean pain was 3.4 the day after the procedure and 1.6 at day 7. Pain scores were no different from haemorrhoidal artery ligation at 21 days and at 6 weeks 9. A trial of banding versus artery ligation reports those scores. The abstract does not name the scale, so treat 3.4 and 1.6 as the trial’s own points, and use them to see that pain was lower at one week than the day after.

A review of banding complications says mild pain is the most common problem, and that delayed bleeding usually arrives 10 to 14 days later. People on antiplatelet or anticoagulant medicine have a higher risk of that bleed 10. Walk if you feel well. Ask the clinician who placed the bands before you return to heavy lifting, especially through that second week.

After excisional surgery

The surgeons’ patient page says pain can last, and that it may be 2 to 4 weeks before you resume your full activities. Most colon and rectal surgeons want a bowel movement within the first 48 hours. The tools they name are a high-fiber diet, a fiber supplement, more liquid, and a laxative if that does not produce a stool 4. A review of pain care after excisional hemorrhoidectomy also recommends laxatives after the operation, along with ordinary pain medicine 11.

Soft stool is the training plan for the first days. Straining on a fresh wound hurts, and it is the push you are trying to retire. Short walks come before loaded squats. Full sessions wait until you can move without holding your breath, inside that 2 to 4 week window or later if your surgeon says so 4.

On the next ordinary day, take the walk and skip the breath-hold. If you are inside the first two weeks after banding, or the first month after surgery, ask the clinician who treated you before the heavy sets come back.

Frequently asked questions

Can I work out during a hemorrhoid flare?

A walk is the activity the NIDDK connects to constipation care, and constipation is one of the habits tied to hemorrhoids. Keep the walk if it does not worsen pain. Pause heavy lifts and any set where you hold your breath and bear down. A French survey found that a harder-than-usual bout of activity in the prior two weeks was more common in people having a flare.

Does cycling cause hemorrhoids?

In a questionnaire of 312 active adults, 57 percent of people who cycled or rode horses said they had hemorrhoids. After the researchers adjusted for other factors, age and bodybuilding were the links that stayed significant, and cycling was not. If the saddle hurts during a flare, skip the ride until sitting is comfortable. The study cannot prove the bike caused the hemorrhoids.

When can I lift weights after rubber band ligation?

The banded tissue most often falls off within a week. In a large trial, pain after banding averaged 3.4 the next day and 1.6 at day 7, and it was no different from a comparison procedure by 21 days. Delayed bleeding is most often 10 to 14 days after banding, especially if you take blood thinners. Ask the doctor who placed the bands before you return to heavy lifting.

Is yoga safe when hemorrhoids hurt?

The NIDDK constipation page does not list a yoga routine. It lists regular activity, and biofeedback if the muscles that pass stool are not working together. Skip poses that have you brace your belly and hold your breath while you are sore. Walking is the activity named for constipation.

How soon can I exercise after hemorrhoid surgery?

The colon and rectal surgeons' patient page says it may be 2 to 4 weeks before you are back to your full activities. Pain is expected, and the first goal is a bowel movement within 48 hours, using fiber, fluids, and a laxative if the stool does not come. Walk as soon as your surgeon says that short walks are allowed. Save heavy lifting for after the pain has settled and you are not holding your breath to get through a set.
