Quick answer
Hemorrhoid itch feels worse in bed because of warmth, sweat, and smears of stool on the skin. A review of anal itch calls nighttime itch a sign of that seepage and says washing it off helps most. Hydrocortisone for piles and itchy bottom should not be used for more than 7 days. If wipes make the itch worse, stop them.

Key takeaways
- Anal itch, called pruritus ani, affects about 1 to 5% of people. In reviews, hemorrhoids are the anorectal problem found most often alongside it.
- A fecal patch test itched within about 6 hours and improved with washing, which points to irritation from stool rather than an allergy to it.
- Heat and sweat make the itch worse. In bed, that lines up with warm covers, and people scratch in their sleep without meaning to.
- Wet wipes preserved with methylisothiazolinone have caused perianal rashes that cleared after the wipes were stopped.
- NHS advice is not to use hydrocortisone treatments for piles and itchy bottom for more than 7 days. If the itch is no better, or it returns quickly, see a doctor.

Hemorrhoid itch often gets worse at night because the skin is warm, a little damp, and easy to scratch once you are still. Anal itch has a name, pruritus ani, and the reviews of it describe nighttime itching as a clue that a small amount of stool is sitting on the skin. Washing that off helps more than another layer of cream 1.
An external hemorrhoid makes cleaning harder, so stool and mucus stay in the folds. The itch is the skin reacting to that moisture. This page covers why night is worse, what the pruritus ani research actually measured, how wipes cause rashes, and how long anti-itch medicine is meant to be used.
NIDDK lists anal itching as a hemorrhoid symptom, and says that too much straining, rubbing, or cleaning around the anus can make symptoms worse. For many people, external hemorrhoid symptoms ease within a few days once that irritation settles 10.
Why the itch gets worse at night
Siddiqi’s review of pruritus ani is direct about the night problem. Acute itch is a marker of fecal seepage, and washing the area right away is the most effective remedy, especially for nocturnal itch 1. You notice it in bed because you are still, and because the trigger is still on the skin.
Heat and sweat make itch worse. Siddiqi found no solid evidence that clothes cause pruritus ani, and still advises underwear that lets air through so the skin stays dry 1. Loose cotton at night follows that advice.
Itch from many skin diseases flares at night, and anal itch fits that pattern. Patel, Ishiuji, and Yosipovitch’s review of nighttime itch found it reported across many conditions, with worse sleep. The mechanisms are not well worked out. They discuss the daily rhythm of itch-related chemicals, and changes in skin temperature and barrier function. Treatments aimed at nighttime itch are scarce 3. The paper is about itch in general. It explains why an already itchy patch often feels louder after dark.
Sleep makes the scratching worse. Ortega and Delgadillo write that people who are asleep cannot resist the urge to scratch the way they can when they are awake. Once the skin is broken, scratching hurts, and burning and bleeding can follow 2. A sedating antihistamine is sometimes used here. Siddiqi says systemic antihistamines may reduce nocturnal scratching, that no randomized trial has tested them for pruritus ani, and that a sedating antihistamine may work by helping you sleep rather than by blocking itch in the skin. Topical antihistamine creams are too weak for this and can sensitize the skin 1. Ortega specifically mentions oral diphenhydramine at bedtime for people who are worst during sleep 2. Ask a doctor or pharmacist before you take one, and follow the label. Do not put antihistamine cream on the anus.

What pruritus ani research found
Itching that is not caused by hemorrhoids, including pinworms, skin conditions and food triggers, is covered in itchy anus: causes and how to stop the itch.
Pruritus ani affects about 1 to 5% of people, about four times more often in men, most often between the ages of about 40 and 60 1.
The usual driver is stool on the skin, including amounts too small to notice. In patch testing with a person’s own feces, itch showed up in a third of people who already had pruritus ani and in 53% of people without symptoms. Itch mostly started within 6 hours and got better with washing, which fits irritation rather than allergy. Only 4% developed irritation when the same test was done on the arm. Perianal skin reacts differently from skin elsewhere 1.
Loose stool makes seepage more likely. In the studies Siddiqi reviews, 50% of people with pruritus ani had loose stools, and 41% of that group reported soiling at least weekly. Up to 52% of people with pruritus ani have an anorectal disease, and hemorrhoids are the most common one 1.
Daniel and colleagues examined 109 patients whose presenting symptom was anal itch. Mean age was 52.1 years, and men outnumbered women 2 to 1. Symptoms had lasted a mean of 6.1 weeks. Hemorrhoids were found in 20% and anal fissures in 12%. A quarter (27 of 109) had no other anorectal disease. Treatment helped 89%, and 11% stayed symptomatic 4. Every patient in that clinic was scoped, so this mix is what a specialist sees, not one itchy night at home.
Ortega and Delgadillo call fecal contamination the common thread, and they describe the itch-scratch cycle: stool and bacterial enzymes irritate the skin, scratching breaks it, and the broken skin itches more. Flatulence can deliver a fine spray of that material. Soaps, aggressive wiping, and shaving the area can escalate a problem that started as a cleanliness effort. Chronic use of topical lidocaine can sensitize the skin or cause a chemical dermatitis 2.
Coffee, alcohol, citrus, tomato, spices, and chocolate are often blamed. Some reports described less itch within 14 days of cutting them, and controlled trials have not shown that this works. The reviews still suggest a two-week trial of one food at a time, because these foods can loosen stool 12.
Surgery is a weak itch treatment on its own. Ortega writes that pruritus ani is not a reason for hemorrhoidectomy, because removing hemorrhoids may not stop the itch. A large skin tag is different: it gets in the way of cleaning, and removal can be considered if gentle washing has failed 2. Tags and outside swelling are covered with external hemorrhoids. Symptom meanings are on hemorrhoid symptoms.
How wipes and creams can start a rash
Contact dermatitis on the anus looks red, scaly, and sometimes blistered. Siddiqi lists sensitizers in cleansers and treatments: creams, soaps, wet wipes, glyceryl trinitrate, anesthetic creams, and toilet paper dye. In a patch-test study described in that review, 69% of pruritus ani patients reacted, and 38 of 55 reactions were to products they were using as treatment 1.
If the itch gets worse after wet wipes, Siddiqi says to stop them immediately, in case you have become sensitized 1.
The wipe preservative methylisothiazolinone has case evidence behind it. Chang and Nakrani reported six children with chronic perianal, buttock, and facial dermatitis that had not responded to antibiotics and corticosteroids. All were patch-test positive to methylchloroisothiazolinone/methylisothiazolinone. None were in diapers. All had been using wet wipes that contained methylisothiazolinone alone. Stopping the wipes led to rapid, complete clearing 5. Admani, Matiz, and Jacob reported perianal dermatitis in a child from the same preservative in wet wipes, and they describe methylisothiazolinone as a moderate to strong sensitizer 6.
Adults get this pattern too. In a Spanish patch-test series, 16 of 37 patients with perianal eczema had a positive patch test. Methylchloroisothiazolinone/methylisothiazolinone was the main allergen, and sensitization often came from wet wipes 7. The series are small. A wipe that feels gentler than dry paper can still keep the rash going.
The best hemorrhoid wipes page is for people who still want a softer clean during a flare. If your itch began with wipes, skip that aisle until the skin has calmed, and use water instead.
How to keep the skin clean and dry
Siddiqi’s cleaning steps are what clinicians recommend. The review is clear that controlled trials of hygiene for pruritus ani do not exist 1.
- After a bowel movement, wash with water. Skip soap on the anus. A shower head, a bidet, or a sitz bath all work. A portable bottle is the away-from-home version 12.
- Dry the skin before you get dressed. Siddiqi mentions a hair dryer on a cool setting, or a soft pat. The NHS piles advice is the same idea: keep the area clean and dry, and pat rather than rub 111.
- Put on a barrier after washing. The review names zinc oxide or petroleum jelly. For cleaning outside the house, it names a small tube of an oil-based cleanser (Balneol) or aqueous cream on cotton 1.
- Wear cotton underwear, and put it on only when the skin is dry. Change it daily 1.
- At night, wash off any smear before you lie down, then use the barrier. That is the nocturnal-itch step, not a heavier steroid 1.
Warm soaks help the hemorrhoid soreness and rinse the skin at the same time. Time and temperature are in how to take a sitz bath. Toilet posture and gentle cleaning are in toilet habits for hemorrhoids.

Ortega adds a few limits: skip soaps that dry or sting the skin, and do not shave the anal margin, which can set off intense itch 2. Cutting back on alcohol, tobacco, and caffeine is reasonable. A food trial has not been proven 2.
A barrier ointment for raw, itchy skin and a cleanser you can carry are the two products that match this routine. Buyer notes on both are summarized on the best hemorrhoid ointments page and the wipes page above.
Raw, itchy skin from frequent wiping or loose stools
Anti-itch creams and how long to use them
Use a steroid for a short, planned stretch, then stop.
The NHS page on hydrocortisone for piles and itchy bottom says you will usually apply these treatments several times a day: morning, last thing at night, and after a bowel movement. Creams, ointments, and sprays are for outside piles and itchy bottom. Suppositories are for internal piles. Do not use hydrocortisone treatments for more than 7 days. Book a doctor visit if you are no better after 7 days, or if symptoms come back quickly 8.
NIDDK gives the same one-week frame for store-bought hemorrhoid creams, ointments, and suppositories used for mild pain, swelling, and itch. Follow up if they have not helped after a week, or if they cause dry skin or a rash 9.
Siddiqi’s review, written for clinicians, uses a slightly different clock. Mild itch can be treated with 1% hydrocortisone morning and night after washing, then replaced with a barrier as soon as the skin eases. Stronger steroids, for worse skin changes, should stop by 8 weeks unless a specialist says otherwise. The longest safe run of 1% hydrocortisone has not been established. Potent steroids can thin the skin, and stopping them can cause rebound itch 1. Ortega warns that long-term steroids can atrophy the skin and leave itch that is very hard to clear, so they describe short courses as second-line, after hygiene 2.
For a hemorrhoid flare at home, follow the 7-day NHS and NIDDK limit 89. If a clinician prescribes a stronger steroid, use it for the number of days they name, then switch to the barrier. Rebound itch the day you stop is described in the reviews, so do not restart a potent steroid on your own 12.
Numbing creams and steroid creams do different jobs. The comparison, including how long each is used, is in anti-itch or numbing cream. Chronic lidocaine on this skin can itself become a cause of dermatitis 2.
Pregnancy and breastfeeding need a separate check. Staroselsky and colleagues, answering a clinician’s question about third-trimester hemorrhoids, wrote that commonly used topical products have not been assessed for safety in pregnancy, that harm to a third-trimester baby from the anesthetic, corticosteroid, and anti-inflammatory components is unlikely, and that most women’s symptoms ease after birth 12. That is not a personal clearance. Ask your obstetric provider, and use the pregnancy hemorrhoid guide for the options that are usually discussed.
When night itching needs a doctor
A few itchy nights during a known hemorrhoid flare can be handled with the steps above. Itch that lasts for weeks, spreads, or comes with bleeding needs an exam. Daniel’s clinic found hemorrhoids, fissures, and other disease behind the same complaint 4. Siddiqi notes that fungal infection accounts for up to 15% of pruritus ani in some series, and that a sharp-edged rash can be psoriasis or another skin disease 1. Hemorrhoid cream does not treat those rashes.

When to see a doctor for hemorrhoids: red flags, which clinician to book, what the exam includes, how to prepare, and telehealth limits.
If the 7-day steroid course is finished and the itch is back, look first at wipes, soap, and moisture. Wash, dry, and use a plain barrier for the next several nights. If that week is no better, book the exam rather than starting the steroid again 18.
Frequently asked questions
Does nighttime itching mean the hemorrhoid is getting worse?
Nighttime itching can happen while a hemorrhoid is stable. Itch often peaks when you are still, warm, and not distracted, and when a little stool is sitting on the skin. A hemorrhoid can be the reason the skin is hard to clean. A new hard painful lump is a different problem from an itch that shows up when you get into bed.
Can flushable wipes cause anal itching?
They can. Dermatitis reviews list wet wipes among the products that sensitize perianal skin. In patch testing of people with perianal eczema, methylchloroisothiazolinone and methylisothiazolinone were the main allergens, often from wipes. If the itch started or worsened after you switched to wipes, stop them and rinse with water.
What can I use at night if I cannot use steroid cream every day?
After a short steroid course, switch to a plain barrier such as zinc oxide or petroleum jelly, which pruritus reviews suggest once the skin is calming down. A sedating antihistamine at bedtime is sometimes prescribed for people who scratch in their sleep. There is no randomized trial of antihistamines for anal itch, so this is a doctor or pharmacist decision, and you should follow the label.
Why do I scratch the area in my sleep?
Once the itch-scratch cycle is going, sleep does not turn it off. A review notes that people scratch while asleep because they cannot resist the urge the way they can when they are awake. A sedating antihistamine may help by making you sleep more heavily. Cleaning a smear of stool before bed attacks the reason the skin is itchy.
Can I use hemorrhoid cream for itch while pregnant?
Check with your obstetric provider before you start. A clinical review notes that common topical hemorrhoid products have not been formally tested for safety in pregnancy, and that the anesthetic, steroid, and anti-inflammatory pieces are unlikely to harm a third-trimester baby. Many pregnancy hemorrhoid symptoms ease after birth. Do not stretch a steroid cream past a week on your own.
